  1. 1. DOCUMENTATION REQUIRED FOR FOSTER/ADOPT FAMILY VERIFICATION<br /><ul><li>Foster Parent Documentation (one copy per foster parent, unless otherwise noted)
  2. 2. Driver’s License
  3. 3. Social Security Card
  4. 4. Background Check (includes FBI fingerprint verification)
  5. 5. Request for Criminal History (both c and b are the same form. After completing the form, return to Jonathan’s Place. The next steps will be taken from there.)
  6. 6. Authorization for Release of Information
  7. 7. Affidavit (Please note, the affidavit will need to be notarized.)
  8. 8. Personal References (3 for each person; may submit 3 that are for both parents. Please use the form from Jonathan’s Place.)
  9. 9. Medical Clearance Form/Letter
  10. 10. Medical History Form
  11. 11. TB Test Result (both items j and k can be done at your choice of place. The specification need for the drug test is called a, “pre-employment drug test.”

You must make the contact and document on the form the result of your contacts, even if the fire and/or health requests a call/verification from Jonathan’s Place. After you have recorded the result of your efforts of contacting the fire and health inspectors, and/or have completed both inspections, item d has also been completed.)
