International Congress of Oral Implantologists (ICOI), defines the surgical guide as-(dental implantology update oct 2009) A guide , derived from the diagnostic wax-up, used to assist in the preparation for and placement of dental implant, it dictates drilling position and angulations. Synonyms- Surgical template, Surgical guide, Surgical device, Surgical jig, Implant positioning device or Surgical stent.
  6. 6.  Ideally the surgical guide should possess the following characteristics-(dental implantology update aug 2009) 1-Simple and cost effective to fabricate, 2-Stable and retentive in surgical field 3-Easy access of drill / guide pins or osteotomes intra-operatively; 4-Ability to translate pre-surgical work up information accurately to operating field.

Technique – Duplication of the mandibular complete denture,by polyvinyl siloxane impression material, Use the impression as a mold for the radiographic guide, Radiographic guide is prepared by mixing Barium Sulfate powder(10 to 15% by weight) with acrylic resin powder before incorporating the monomer,
  8. 8. Index desired emergence implant sites in the position of the lateral incisor, canine and first premolar areas, Drill 6 grooves with a 2mm twist drill parallel to each other,
  9. 9. Add autopolymerizing acrylic resin on the occlusal surface of the duplicate prosthesis to create an accurate record of the occlusion and to ensure correct placement of the radiographic guide, Select optimal implant site from the radiographic images
  10. 10. Drill the guide according to the planned implant position and orientation with 3 mm twist drill with an internal diameter of 2.1 mm in the selected position to serve as drilling guide,
  11. 11. Use the surgical template to guide the 2mm twist drill through the cortical bone for each implant site, direction can be checked by direction indicator
  12. 12.  Used as a custom tray- surgical template can be used as custom tray for final impression after modification of the intaglio surface of the duplicate denture
  13. 13.  Advantages – 1-There is no damage to the existing prosthesis, 2-Cost effective 3- Material for fabrication easily available, 4-Easy to fabricate, 5-Can be easily used for resorbed ridges.
