Purpose of a form • Workshop, conference, event registration • Membership registration • RSVP for an event • Enquiry/Contact form – also on web • Opinion poll/Survey • Evaluation/Assessment, Exam, Test • Competition, raffle, donation, fund raising, interactive story • Order, shopping chart, wish list • Research survey to collect research data • ILLs, Report lost book/item, comments, Ask a Librarian, Venue bookings, etc.

Using the form • Web address/link/URL will be generated by Google Forms • Communicate link to participants: • From Google Forms • Via email • Embed as part of web page • More user friendly to hyperlink text, e.g.: Register for the workshop – click here. • Shorten URL generated using: goo.gl, bit.ly, tinyurl
  10. 10. Requirements • Gmail address e.g. heligliasa@googlegroups.com • Login with profile in which you want to create form • Login at: http://gmail.com • Username • Password • Then access http://forms.google.com

Planning the form (1) • Professional branding • Spelling & Grammar • Test form before inviting participants to complete • Can be completed anonymous • Mandatory & optional fields • What kind of info do you require? What is put in by respondent is what you will get out.” • Design form in such a way that data collected will be easy to analyse within Excel

Planning the form (2) • Length, audience, purpose of form • Pages: Introduction, Questions etc. • Indicate purpose of form, due date for completion • Indicate number of questions, time it will take to complete • Contact details • Logical flow between questions • View existing examples to assist you when compiling your form • Ethical clearance
