This includes names, dates, and number. </li></ul><ul><li>Name must be EXACTLY as it appears in the passport. </li></ul><ul><li>5 blank VISA pages or boarding will be denied. </li></ul><ul><li>Expiration date is 6 months AFTER you arrive back in the United States. </li></ul>
