  2. 2. <ul><li>Good News This Year!! </li></ul><ul><li>There will be no plan or rate changes to any of your Health, Dental, Basic Life and Disability plans. </li></ul><ul><li>Dependents up to age 26 are eligible to be on your plan whether or not they are married, living with you, in school, or financially dependent on you. </li></ul><ul><li>Preventative care visits are covered at 100%...You pay nothing! </li></ul><ul><li>Enrollments can now be completed online. </li></ul><ul><li>Full-time employees will now have the option of purchasing voluntary life insurance to supplement their employer-paid basic life insurance benefit during the open enrollment period. </li></ul>
  3. 3. How to Enroll <ul><li>If you are a full-time employee and not currently enrolled in UAC’s medical benefits or want to enroll a spouse or dependent(s): </li></ul><ul><li>Please contact the Human Resources Department for appropriate enrollment forms. </li></ul>

How to Enroll (con’t) <ul><li>If you are not making any changes to your current plan elections please follow these steps to participate in Open Enrollment online: </li></ul><ul><li>Log on to the https:// portal.adp.com  Web site. </li></ul><ul><li>Complete the following fields on the Login page: </li></ul><ul><ul><li>Select User Login </li></ul></ul><ul><ul><li>Enter Login/User Name </li></ul></ul><ul><ul><li>Enter Password </li></ul></ul><ul><li>Click OK to proceed to the Welcome screen. </li></ul><ul><li>Click on the My HR and Benefits link within the Quick Links section of the portal and select Employees and Manager , Benefits and then Review/Change Benefits . </li></ul><ul><li>A wizard-based Enrollment Tool begins the Enrollment process. The following three benefit enrollment options are available: </li></ul><ul><ul><li>Walk me through this process </li></ul></ul><ul><ul><li>I know the changes I want to make </li></ul></ul><ul><ul><li>Review my benefits coverage </li></ul></ul><ul><ul><li>Select “Walk me through this process”. </li></ul></ul><ul><ul><li>Click on “ Continue ” to proceed to the next plan option until you reach the Benefits Summary page and click on “ Accept with No Changes ”. Please note, you will have to re-enter your FSA deduction amount even if you wish keep the amounts the same. </li></ul></ul>

How to Enroll (con’t) <ul><li>If you wish to change your current plan elections please follow these steps to participate in Open Enrollment online: </li></ul><ul><li>Log on to the https:// portal.adp.com  Web site. </li></ul><ul><li>Complete the following fields on the Login page: </li></ul><ul><ul><li>Select User Login </li></ul></ul><ul><ul><li>Enter Login/User Name </li></ul></ul><ul><ul><li>Enter Password </li></ul></ul><ul><li>Click OK to proceed to the Welcome screen. </li></ul><ul><li>Click on the My HR and Benefits link within the Quick Links section of the portal and select Employees and Manager , Benefits and then Review/Change Benefits . </li></ul><ul><li>A wizard-based Enrollment Tool begins the Enrollment process. How to Enroll (con’t) <ul><li>If you wish to delete a dependent or spouse from your current plan elections please follow these steps to participate in Open Enrollment online: </li></ul><ul><li>Log on to the https:// portal.adp.com  Web site. </li></ul><ul><li>Complete the following fields on the Login page: </li></ul><ul><ul><li>Select User Login </li></ul></ul><ul><ul><li>Enter Login/User Name </li></ul></ul><ul><ul><li>Enter Password </li></ul></ul><ul><li>Click OK to proceed to the Welcome screen. </li></ul><ul><li>Click on the My HR and Benefits link within the Quick Links section of the portal and select Employees and Manager , Benefits and then Review/Change Benefits . </li></ul><ul><li>A wizard-based Enrollment Tool begins the Enrollment process.
