EPIC Primary Care has been designated an ‘Advanced Patient
Centered Medical Home’ since 2011. The National Committee
for Quality Assurance confers this recognition only on Practices
that meet certain criteria.
According to the NCAQ, the Patient Centered Medical Home is
“model of care that puts patients at the forefront of care.
PCMHs build better relationships between patients and their
clinical care teams.”
