 Using questionnaires
 Conducting an interview
 Contacting health surveillance assistants
 Community leaders
 Epidemiological survey data
 Community’s health facility documents/data
 Identify and assemble a diverse community team
 Develop a team strategy
 Define community to assess (e.g., region, village)
 Identify community sectors to assess (e.g., health care, schools)
 Identify community components to assess (e.g., nutrition, tobacco
 Develop questions to ask for each community component
 Select sites and number of sites to visit within each sector
 Determine existing data to use or methods for collecting new data Identify key
informants to contact
 • Develop a rating scale,
 • Make a team decision on ratings
Enter the data,
 Total the data,
 Summarize the data.
• Identify community assets and needs,
• Prioritize needs,
• Develop and prioritize strategies for
improvement,
• Create an action plan for top priority
strategies
As the name applies, comprehensive needs assessment is the most
thorough assessment of the community, it is also the most
traditional and the most time consuming.
In the comprehensive approach, the nurse begins with the total
community( geopolitical/ phenomenological) and uses a systematic
process to assess and the aspects of the community to identify or
validate actual and potential health problems.
It is more narrow in scope because it only focuses on one
population.
It involves studying data already available about a community
(e.g. Census, tract data, survey)
This approach helps the community or public health midwife
to focus on a special populations(groups) that have similar
characteristics and may have health care needs.
Information from this data indicates that many children and
adolescents leave in this census tract, having identified the
youth as a target population, the community health nurse
would focus on the assessment of this aggregate.
Some community health midwives may be employed by a health
department or non-profit organizations to work with single
population.
The community midwife assesses on population in the community
for example women of reproductive ages, teenagers and migrants.
The midwife begins the process with defining the population and
then assess the population in a specific community. Often the
population has already been determined to be an especially
vulnerable one.
The community health midwife assesses a community in relation to
specific topic or health problem.
The midwives begin the process with the problem or the topic area and
often assesses a specific community in relation to that subject
Community resources such as city offices, hospitals, community leaders
and health professionals will the targeted to expand the coalition so as
to increase participation and expand services to seniors.
Also the community seniors were targeted with the effect of reducing
diseases and disabilities in this vulnerable population.

  • 9.
  • 10. As the name applies, comprehensive needs assessment is the most thorough assessment of the community, it is also the most traditional and the most time consuming. In the comprehensive approach, the nurse begins with the total community( geopolitical/ phenomenological) and uses a systematic process to assess and the aspects of the community to identify or validate actual and potential health problems. It is more narrow in scope because it only focuses on one population.
  • 11. It involves studying data already available about a community (e.g. Census, tract data, survey) This approach helps the community or public health midwife to focus on a special populations(groups) that have similar characteristics and may have health care needs. Information from this data indicates that many children and adolescents leave in this census tract, having identified the youth as a target population, the community health nurse would focus on the assessment of this aggregate.
