  4. 4. Guidelines that will determine the cost of your health care What is a sliding fee scale? If you are NOT eligible for Medicaid, Healthy Start, or health care coverage through your employer, the health care providers in this book will use a sliding fee scale to decide how much to charge you. The lower your income is, the less you will have to pay. As the income of the household increases, the required family contri- bution increases. What is “Poverty?” aid please call Many sites use the Federal Poverty Guidelines as a way to decide how much a person can contribute to their health care. These guidelines are based on the number of people in a house- hold and the combined income of the members of the household. The following chart shows the Federal Poverty Guidelines. 2007 Poverty Guidelines Number of People in Household 100% 150% 200% 1 $10,210 $15,315 $20,420 2 $13,690 $20,535 $27,380 3 $17,170 $25,755 $34,340 4 $20,650 $30,975 $41,300 5 $24,130 $36,195 $48,260 6 $27,610 $41,415 $55,220 7 $31,090 $46,635 $62,180 8 $34,570 $51,855 $69,140 For each additional person, add $3,480 To get updated information about the poverty guidelines, use the following website: http://aspe.hhs.gov/poverty/poverty.shtml 3

  8. 8. A Guide to Free or Affordable CHILD Health Care for Children Every parent wants to raise a healthy child. With that goal in mind, it is important for all parents to consistently provide their ** Hepatitis A vaccine is recommended for children and teens in regions that have high rates of Hepatitis A. children with preventive health care. Preventive health care can protect your child from harmful diseases and illnesses. Children begin to develop health behaviors and habits very early in life. Therefore, parents have the responsibility of fostering positive health choices and decisions for their children. You should be sure to follow an approved immunization schedule; the chart to the left (Table 1) can serve as a guide. Your child should receive up to eight exams during the first two years. After the second year, your child should receive an exam annually. During each exam, please talk to the doctor about any questions or concerns that you may have. If for any reason you do not feel comfortable with a physician you have chosen, consult with your health care plan to resolve your concerns. This section of the guide will provide you with a list of primary care health services that children need in order to prevent serious health problems and to stay healthy. However, under certain circumstances changes to the recommended health services and schedules may be required. Please consult with your physician to address the individual needs of your child. 7 •
  9. 9. Guidelines for Children’s Oral Health Why is oral health important for my child? Just as adult’s teeth decay, so do baby’s teeth. Although baby’s teeth will fall out, it is important to protect them from cavities and decay. Baby teeth save space for permanent teeth and helps guide them in, so prob- lems with baby teeth can harm permanent teeth. Baby teeth also help kids speak properly, bite, chew, smile, and kiss. How can I afford this? Medicaid and Healthy Start cover dental care. You should bring your child to a dentist for a check up twice a year. If you aren’t eligible for either of these programs and you do not have dental coverage through your employer; see the section on dental options in the chart “Other Free and Low Cost Services” on page 2. Before teeth come in… You can start a prevention regimen well before your child’s teeth come in. Once your baby is born, but before teeth begin to grow, clean the gums and tongue each day with a moist cloth. If your infant uses a bottle to calm down or fall asleep, use water instead of milk or juice to prevent the gums and teeth from decaying. A pacifier is an even better option, but don’t dip the pacifier into sugar or a sugary liquid. Once teeth come in… The first tooth will come in around 6 months after birth, but each child is different and it might take longer in some cases. You should teach your child to drink from a sippy cup by his or her first birthday. Somewhere between age 18 months and 2 years, get a small, soft, nylon tooth brush. Brush each part of each tooth at least ten times. Until age 6-8, help your child brush twice a day so that he or she will adopt this habit. General guidelines to remember for children age 2 and up… • Brush at least twice a day • Use Dental Floss every day • Eat a balanced diet • Visit the dentist twice a year 8

  34. 34. Preventive Care for Teens In today’s society many teens are faced with difficult choices which can lead to health crises such as STD’s, HIV infection, alcohol and drug abuse, unexpected pregnancy, obesity and eating TEEN disorders. Due to the increasing number of these health crises among teens, health care providers have become committed to provide health services specific to the needs of teens. Health care centers in Cuyahoga county offer a variety of health services to help teens establish and maintain good habits for a healthy mind and body. The table to the left (Table 2) reflects recommended preventive health services teens should receive annually. Ensuring that you receive preventive health services can help you avoid disease and illness. Preventive health services can also detect and treat illnesses early on in order to avoid worsening conditions. This section of the guide lists five sites that specialize in teen health services and resource information regarding services for pregnant teens. Notes for Table 2: * Doctors recommend a monthly self-examination, but a clinical testicular exam could be provided with a comprehensive unclothed physical exam. ** A Pap Test and pelvic exam is recommended every three years for women between the ages of 20 and 65. (American Family Physician, March 15, 2001 as published by The American Academy of Family Physicians) *** High risk only: A tuberculosis test should be performed on teens who have been exposed to active TB, have lived in a homeless shelter, have been incarcer- ated, have lived in an area endemic for TB, or currently work in a health care setting. A hepatitis B vaccination should be given to susceptible teens at high risk for infection. 33

These children also have more childhood health problems and are frequently hospitalized. TEEN Access to health care for a mother and her unborn child is an important factor in having a successful and healthy child birth. Proper health care access, such as having a complete exam early in your pregnancy, can prevent unwanted health problems. Lack of transportation and convenient access to health care facilities are often problems for teens. It may also be challenging to find resources while balancing school and motherhood.
