  2. 2. Prescriptive Authority <ul><li>It was the nurse practitioners who initiated prescriptive authority. Given that the patients that the early NPs saw needed medications, waiting for physician derived prescriptions created problems in the areas of access, continuity of care, and patient flow. </li></ul>
  3. 3. Prescriptive Authority <ul><li>It was a long journey for NPs to be able to prescribe. Legislators and regulators had to be convinced and state statutes and regulations had to be changed. </li></ul>
  4. 4. Prescriptive Authority <ul><li>States moved to change their laws at various speeds. Those states with a shortage of primary care physicians moved quickly to enact the new statutes. </li></ul><ul><li>Speed was also enhanced in the rural areas of the country. </li></ul>
  5. 5. Prescriptive Authority <ul><li>Programs of study had to show that they were preparing NPs to prescribe: </li></ul><ul><li>Pharmacology and pathophysiology were added as was clinical decision making and diagnosis skills were embellished. </li></ul>

Prescriptive Authority <ul><li>Ways to authorize prescriptive authority </li></ul><ul><li>1. amend nurse practice act </li></ul><ul><li>2. develop new statutes separate from nurse practice acts </li></ul><ul><li>3. amend states’ administrative codes through the development of regulations promulgated by the appropriate regulating board. </li></ul>

Patterns of Prescriptive Authority <ul><li>Established Formulary that contains agreed-on list of drugs that NPs can prescribe </li></ul><ul><li>Exclusionary Formulary that contains a list of drugs that NPs cannot prescribe </li></ul><ul><li>Collaborative Formulary that contains drugs that NP and Collaborating Physician agree is acceptable for NP to prescribe </li></ul><ul><li>Open Formulary is the most flexible and allows for no exclusions. </li></ul>

Barriers to Prescriptive Practice <ul><li>Organized lobbying by medical community </li></ul><ul><li>Limitations set n state statute and regulation </li></ul><ul><li>Mail order pharmacies often require physician’s signatures </li></ul><ul><li>Confused with other health care providers such as physician assistants </li></ul>

Statistics <ul><li>Prescriptive Patterns reflect practice setting and specialty </li></ul><ul><li>Wrote 19 prescriptions a day if FNP </li></ul><ul><li>Years of experience increased the number of prescriptions written </li></ul><ul><li>Drugs most often prescribed included antimicrobials, anti-inflammatories, and analgesics </li></ul>
