Step 1: Choose the Data ◦ Data sets and maps have been selected from the following sources and analyzed to create a narrative to those communities which have been disproportionally impacted by Cardiovascular Disease and Diabetes:  Bernalillo County Health Assessment, Collective Impact for Neighborhood and County Health.  New Mexico Indicator-Based Information System (NM IBIS).  The Journal of Family Practice, Building Cultural Competency for Improved Diabetes Care (Asian Americans and Diabetes, African American and Diabetes, Latino Americans and Diabetes)  Place Matters for Health in Bernalillo County, Ensuring Opportunities for Good Health for All. A report on Health Inequalities in Bernalillo County, NM.  International District Sector Development Plan and Mountain View Sector Development Plan.

Step 2: Create the Story  Infant Mortality: ◦ Perinatal conditions account for more than half of infant deaths in Bernalillo County, followed by congenital malformations and medical conditions. The perinatal is the period shortly before birth and after birth; from the 20th to 29th week of gestation to 1 to 4 weeks after birth. The correlation between the income of neighborhoods and low birth weights appears weak with the limited amount of current data. ◦ There is a strong correlation to low birth weights and proximity to highways, encompassing exposure to diesel fuels and polluting industry. According to the Center for Disease Control, environmental exposures can contribute to infant birth outcomes especially when combined with stress due to neighborhood conditions and racism.  CVD and Diabetes: ◦ Two of the leading causes of deaths in Bernalillo County are cardiovascular disease (CVD) and diabetes. The communities of Mountain View and the International District have been identified as having disproportionately high levels of deaths for those two particular issues. African Americans, Native Americans and Latinos had double the rate of deaths due to diabetes for the period of 2000 to 2009 when compared to Whites. ◦ Following the framework provided by the Joint Center and Place Matters, one of the most effective ways to tackle health disparities is to address the social determinants of health which include: (Land Use Planning, Economic Development, Housing, Parks and Recreation, Education, Crime, Racism/Discrimination).
  9. 9. Step 3: Develop the Campaign New Study Found that Diabetes is due to strong genetic factors coupled with urbanization and lifestyle changes. Many of the adverse environmental factors are modifiable. Prevention of Type 2 diabetes will require measures to promote physical activity… Task 1. Combine Social Media with Traditional Methods • Crucial to keep networking in the targeted communities to personally invite them to become a social media partner. • Keep people engaged by posting daily. • Invite community members to meetings that will have a positive impact in the community. Some of the meetings include: • Sector Development Plan Meetings, PTA School meetings, American Heart and Diabetes Associations programs and events. • Writing letters to local city councilors, county commissioners, legislators, and congressmen to request support for better policies that promote a healthy community. • Register for classes for healthy eating and lifestyle. • Get screened for diabetes. • Organize community walks. • Enroll in GED and ESL classes.
