Family status Total Out of Pocket Maximum Medical/Vision/Prescription/Dental Coverage: (in-network) – includes • Administered by Aetna deductible and co-insurance • $0 Premium Costs to participate Employee Only $6,000 • Routine preventative care covered at 100% Employee + 1 $12,000 Eligible dependents for Medical/RX/Dental coverage Employee + Family $12,000 include your legally recognized spouse, and dependent children up to age 26.
