SASS can help caregivers and seniors maintain their independence through the aging process. 2. SASS can meet the needs of caregivers and seniors. 3. Individualized service means that service that is a) UNIQUE to the individual, based on his or her preference, needs, concerns and personal priorities b) GOAL-oriented, based on empowering and improving the individual’s abilities in cognitive, social/emotional, physical/medical, adaptive and communicative areas c) STRUCTURED for success, with assessments, short-term goals, and a roadmap to improvement d) SUPPORTED by the team and the staff that work with the person, with support for the person, the family and agencies involved e) OUTCOME-oriented, with accountability built into the plan f) REINFORCED across multiple environments, (home, community, hospital) 4. The person’s best interest is the SASS focus. We want to be the best fit for them but we’ll also connect them to the best resources in their area.
