Market Dynamics Suicide • Suicide now outpace car crashes as the leading cause of death from injury. 1 • 1800 suicides yearly. 2 • 75% of suicides occur in the patient’s bathroom, bedroom, or closet. 3 • 50% of all suicides result in legal claims. 4 • It only takes 4-5 minutes of pressure on the carotid arteries for death to occur, making the current system of 15-minute checks on patients insufficient. 5 • The Joint Commission reports that in 309 of 685 reported suicides, physical environment was one of the root causes. 6

  9. 9. Sentinel Event A sentinel event is an unexpected occurrence involving death or serious physical or psychological injury, or the risk thereof. The phrase, "or the risk thereof" includes any process variation for which a recurrence would carry a significant chance of a serious adverse outcome. Such events are called "sentinel" because they signal the need for immediate investigation and response. 9
