How To Use This Form:

The Property and Facility Survey should be conducted on a weekly basis. Facilitators should start at the lobby, from the first floor, and survey in a clockwise direction. The interior should be done first, then the exterior of the facility. Use a different form for programs with more than one facility. After putting a check or circle around the appropriate color that identifies your general judgment of the items in each category, record location and other identifying notes in the section for other pieces of information. Submit additional copies of this page as needed.
