  1. 1. NOMINATION FORM Sponsored by Transport Canada, the SCL 2010 Green Supply Chain Award salutes supply chain management who have set and exceeded high standards for environmental friendliness. These companies operate in an ethical manner and actively seek ways to improve environmental performance through: identifying goals to reduce the fuel consumption and greenhouse gas emissions from freight facility operations; developing an action plan detailing how each of these goals will be achieved; and reporting progress towards achieving these goals. Recipients of this award are selected on the basis of strong environmental leadership and their firm commitment to promote green shipping standards. ELIGIBILITY: o Nominations are open to all supply chain professionals, including, but not limited to those associated with technology, manufacturing, warehousing, 3PL, marketing, consulting, retail, healthcare and transportation. o Nominees can be from anywhere around the world, and need not be a current member of SCL. o Submissions will be treated as confidential. o Submissions will be reviewed by SCL’s Awards Committee, who will select the winner. o Self-nominations are invited. o Deadline for receipt of nominations by the SCL National Office is March 12, 2010 HOW TO NOMINATE: o Review the eligibility requirements listed above prior to completion of nomination. o Complete one Nomination Form for each nominee. o Nomination forms may be either: e-mailed to yverbitskaia@sclcanada.org or faxed to 905-513-1248 o Submit your completed nomination for receipt no later than March 12, 2010 For more information, contact Yana Verbitskaia at 905-513-7300 or 1-866-456-1231.
