  3. 3. Think about all the tools, equipment and appliancesyou’ve bought in your life. Almost all of thesecame complete with an owner’s manual containinginstructions that explained howto put the item together, howit worked and what couldgo wrong. Many of thesemanuals even ex-plained how totroubleshoot or fixthings if they stop-ped working correctly. So why is it that you don’t get an owner’s manual foryour body — especially your sex organs?Surveys show that in most healthy relationships, the penisissomethingthatisusedalot.Unfortunately,it’snotalwaysuser friendly. You assume your penis will always workperfectly and mostly it does. But sometimes it fails andthat’s scary. The situation can be bewildering, frustratingand at times terrifying for both you and your partner. An owners manual for men2TV INSTRUCTION MANUALGuideHow To GuideReferenceManualEncyclopediaInstructionGuidewww.trebest.webs.com
  4. 4. It seems there is a real need for an owner’s manual forthe penis — to help you when something goes wrongand tell you how to fix it. That is the purpose of thismanual — to tell you, in plain language, some of thethings you need to know. To answer some of yourquestions and lessen some of your fears, and mostly tolet you know that you’re not alone: every man with apenis is in much the same boat. This owner’s manual includes information on:q how your sex organs workq reasons why you might suddenlyfind it difficult to have sexq what your doctor can do tohelp when there’s a problemq how to help your partner understandthe problem and share in solving itq ways to get back to a good sex life.Although this manual may answer some questionsabout problems with sex, if you think you have aproblem, you should always talk with your doctor forpersonal advice.AN OWNERS MANUAL FOR MEN3www.trebest.webs.com
  5. 5. Much as your nose or ears are unique, so is your penis. No two penises are exactly alike, each has its ownpersonality, some are lean and sensitive, some are thickand bulky. The head of one may resemble a fireman’shelmet, another may be round, blunt or cone shaped.Your penis is actually quite hollow — it is made of threetubes which run from the base to the tip.Everyone is different4Structure of the penisTube connecting to thebladder — allowingurine to be passedUrethra (tube allowing urineto pass from the bladder)TestisSpongy tissue(most of the penis ismade of this tissue)Blood vesselTubes of erectile tissue— these fill with bloodduring an erectionBlood vesselCross-sectionof the penisDirection ofcross-sectionwww.trebest.webs.com
  6. 6. q One tube connects to your bladder and allowsyou to pass urine.q The other two tubes are used to make yourpenis hard enough to have sex (an erection). They are filled with a special sponge-like tissuethat swells and becomes firm when blood flowsinto it. This happens when you become arousedor feel like having sex.q There is about seven to eight times more bloodin your penis when it is hard than when it is soft.In a hard penis, the blood normally can’t flowback out. This is because the pressure of theblood in the tissues closes the blood vessels thatwould carry it out to the rest of the body. All thisis controlled by the brain. Once sex is over, themuscle tissue contracts and allows the blood toflow away, back into the rest of the body. You can see that your penis has to be able to changefunction as needed. When at rest it is used to empty thebladder of urine, when aroused and erect (hard) itprovides the way to have sex and is a channel for semen.q Between half and one teaspoonful ofsemen comes out of your penis each timeyou ejaculate (discharge semen).q The semen contains millions of sperm —yet only one is needed to make a baby.EVERYONE IS DIFFERENT5www.trebest.webs.com
  7. 7. q A smaller amount of semen will be released ifyou have a few ejaculations during one sexsession. If you have many hours or days betweenthe times you have sex, then the amount ofsemen should be about the same each time.q Regular sexual activity can keep the body’ssexual systems in good shape. Lack of use cancreate its own problems with sex after a time.q As you may have noticed, it is difficult to urinatewhen the penis is erect. It is also difficult toejaculate semen when the penis is limp. All men are born with uncircumcised penises. If yourpenis remains unchanged, it takes a little more time tokeep the nooks and crannies clean.q Draw the foreskin (hood) back fully.q If it gets stuck on the rim of the penis,or if it is painful and seems to stick in placesa small repair job may be necessary. This simple procedure loosens the foreskinand doesn’t require circumcision.q Remember to replace the hoodin a comfortable position.EVERYONE IS DIFFERENT6www.trebest.webs.com
  8. 8. Nearly every man is concerned about the size of hispenis. In many cultures, potency, maleness and generalstrength are thought to be set by the size of the penis. This is a fantasy because the penis will constantlychange its size depending on the outside temperature, your inside feelings and the circumstances of themoment. Your penis shrinks with fear and cold andappears larger in a warm, comfortable atmosphere. Yet,it is frequently when at rest that men try to comparethemselves. There may be no worse place to try tocompare yourself with others than in a urinal line or agroup shower room.It is almost impossible to judge what the erect size ofyour penis is when it is at rest and limp. In spite of thefact that all penises are physically different, their sizewhen erect and hard is more or less the same.q When erect, the average length of the penisis twelve centimeters (about five inches).q A penis that seems small can swell a great dealto become erect. A seemingly large penis mayswell less. Both types end up more or less thesame size when fully erect.q Size can be an illusion in men of differingbuild. A small, slightly built man may havewhat appears to be a large penis. A large, heavily built man’s penis may look smaller. There are no secrets about size7www.trebest.webs.com
  9. 9. An erection — a hard penis — is simply a sign of comfortand arousal (sexual excitement). It doesn’t take a lot ofarousal to cause an erection, especially for younger men.Usually, men do have erections while they sleep. Theyaccompany dreams which happen three or four timeseach night — whether you remember them or not. Thishappens to women too. When a woman dreams, hersexual organ (the clitoris) swells, similar to whathappens to the penis during an erection.q If your dreams are in any way anxietyprovoking or scary, there will be no erection.q When you wake up with an erection, you arestill in part of the dream cycle. Your erectionhas nothing to do with having a full bladder. It is also not always a sign of arousal.Young men can get an erection very quickly — througherotic stories, pictures or fantasy — or just thinkingabout sex. As you get much older, it takes longer for yourpenis to harden. More time, stimulation or manual helpmay be needed. How it worksThere are both physical and mental ingredients involvedin having an erection. These are parts of yourrelationship that can also affect your sexual health. How an erection happens8www.trebest.webs.com
  10. 10. This is what a man needs to have an erection:q Healthy blood vessels throughout his body,so the blood flows in the right ways.q No nerve damage in the area of the hips, stomach or the actual sex organs.q The right amount of special body chemicals (theones that travel in the blood and affect the sexorgans. For example, those that help theblood stay in the penis during intercourse).q A relaxed and happy outlook, without too much stress.HOW AN ERECTION HAPPENS9What a man needs for an erectionGood nerve supply tothe penis from themain nerves of the hipand backbone areasHealthy blood vessels to carry specialbody chemicals and to control the bloodflow for an erection to be possiblewww.trebest.webs.com
  11. 11. Being sexually aroused is the most important trigger toan erection. It is a step-by-step process that is set inmotion by substances that are always present in thebody, just waiting to be called into action. The process is like this:HOW AN ERECTION HAPPENS10You havea sexual orerotic feelingA special bodychemical is released causingthe muscles in the walls ofyour penis to relaxBlood flows into thespongy tissue of your peniscausing it to harden and become erect. As the inside of your penis fills with blood, the spongy tissue squeezes against veins in thewalls of your penis stopping the blood fromflowing back out. All this blood heldin your penis keeps it hard.www.trebest.webs.com
  12. 12. ...the ejaculation. This is a sudden spurt of semen lastinga few seconds. It can feel differently depending on howintense the arousal was.q If you have frequent sex ending each time withan ejaculation, the strength of your feelings andthe amount of semen may lessen for a while.Once your body has had a rest for a few hours ordays, the amount of semen should return tonormal. Usually a certain amount of semen isproduced by the body each day. The making ofsperm inside the body continues at the same rateas long as a man is in good health. The numberof times a man has sex over his lifetime does notaffect the amount of semen the body produces.Sometimes the intensity of the sexual feeling maybe so great that the muscle twitch may spread to theback passage (anus) causing a short, sharp pain there —this is normal. Some men have difficulty with ejaculation. The semenmay be passing into the bladder and not out of the tip ofthe penis. If this happens, it may need a visit to a doctorfor repairs. Many men ejaculate too quickly. They finish before theirpartnerscanenterthegeneralflowandrhythmofthesexualexperience to enable them to also achieve an orgasm.Lastinglongerisalearnedexperienceandcancontributetoa mutually satisfying relationship with the partner. And the result is...11www.trebest.webs.com
  13. 13. ‘Wet dreams’, or ejaculating during an erotic dreamwhile you are asleep, is normal for all men. There is anormal build up of the semen that the body releases onits own while you sleep.AND THE RESULT IS...12After anejaculation the systemgoes into reverseA differentspecial body chemicalis releasedYour penis returnsto its limp stateThis causes the musclesof your penis to contract or pullinwards. Blood now flows back outof your penis and into therest of your bodywww.trebest.webs.com
  14. 14. All erections are angled sharply upwards. As you growolder your penis gradually sags to become more at rightangles with your body.Your erection will last as long as the sexual stimulationlasts. A sudden distraction may cause you to lose yourerection. If the stimulation is still there your penismight become hard again. Fear or worry may cause anerection to fail.In younger men, both getting and losing an erection canhappen quickly. In older men this is more gradual. As you age, it may become more difficult to reach anerection — and you may lose it before you are ready to.But this doesn’t always happen. There are many seniorcitizens who function very well sexually. Paying attentionto your general health, taking care of your penis, having agood relationship with your partner and a healthyattitude towards sex will all help you as you age.AND THE RESULT IS...13www.trebest.webs.com
  15. 15. Did you know that more than half of all men over 40 havehad a problem with getting and keeping an erection? InAmerica alone, it is thought that over 30 million menmay suffer from this problem. It is a very common experience and is now understoodto be a real medical condition that can be treated. There are many problems — both physical and emotional— that can affect the smooth functioning of your penis.Problem...what problem?14Only 2 millionhave ever hadtreatment (that’sonly one manin every 15).Over100 millionmen (1 in every 10)in the world worryabout not being ableto have an erectionat some point intheir lives.www.trebest.webs.com
  16. 16. Physical problemsAmong the physical problems that may cause you tohave trouble with an erection are:q Agingq Illnessq Accidentsq Smokingq Operationsq Alcohol, andq Drugs (both prescriptions from the doctor and illegal drugs)AgingSometimes, the aging process causesa change in the sexual habits andabilities of men. This doesn’t happento everyone, but changes can occurin the time it takes to get and keepan erection and in the hardness ofyour penis. It’s all perfectly natural, and the solution may be an easyone. If an older man finds it difficult to have sex, extraforeplay and attention may solve the problem. Thiscould be enough to help get your sex life back to normal.PROBLEM...WHAT PROBLEM?15www.trebest.webs.com
  17. 17. The chart below shows some of the differences amongmen as they age.PROBLEM...WHAT PROBLEM?16Younger men can usuallyhave an erection at almost any time, just by thinking about sex. They also usuallyhave a higher sex drive than older menand may recover after ejaculation and beready to have sex again in justa few minutes.Teen to 40A mans response to sexualthoughts becomes slower, and moreforeplay tends to be needed. Manual helpmight be needed to make the penisfirm enough to be ableto have sex.40 – 70An older mans erectionsmay be less firm, and he might findit difficult to have sex again for quitea few hours after a first ejaculation. For men in their 70s, this canbe as long as 12 to 24 hours.Over 70www.trebest.webs.com
  18. 18. IllnessAs men age, there are lots of common diseases that canall cause problems in getting and keeping your penishard enough to have sex. Illnesses such as hardening ofthe arteries, high blood pressure, stroke, heart disease, heart attacks, diabetes (where the body is unableto control the blood sugar level)and injury to your sex organscan all cause problems. If aman has cancer and istreated with chemotherapy, radiotherapy or surgery, this can also causeproblems with sex. Other illnesses such as kidney andliver diseases can affect your sex life, particularly if thosespecial organs begin to fail.Often, when many illnesses start, a man can find itdifficult to have or keep an erection because the bodymakes changes to try to get well again. This can meantaking energy away from sex to help make a man wellagain. For long-term illness, this can be a slow processand the loss of ability in having an erection might noteven be noticed for quite a while.A man can also have problems with the balance of thespecial body chemicals that help sex happen or controlhow a man feels in general. For example, there is aPROBLEM...WHAT PROBLEM?17www.trebest.webs.com
  19. 19. thyroid gland in the body that can be overactive orunderactive, and this can make a man feel restless orvery tired all the time. A thyroid gland problem cancause a man to lose interest in sex because he is notrelaxed enough or doesn’t have the energy. There can be problems with the levels of body chemicalsthat can control how a man feels about sex. The bodychemical that controls sex in men is called testosteroneand this is also what makes a man grow more hair than awoman, have a deeper voice and develop male sexorgans (the penis and testicles). As a man ages, theamount of testosterone in the body nearly always beginsto drop quite naturally. This is one reason why sex driveand ability can decline with age.Accidents and operationsSome men have problems with sex after theyhave had an accident or operation. This is usually because therehas been some damage tonerves or blood vessels.Anything that causes theblood flow into your penisto be reduced can stopyour penis becoming hardenough for sex.PROBLEM...WHAT PROBLEM?18www.trebest.webs.com
  20. 20. Accidents or operations affecting your lower bodymight cause nerve damage that couldaffect your penis and whether you canhave an erection. Breaking bones, suchas your hips or lower back (thepelvic area), or accidents to thespine can also affect thenerves or blood flow andcause erection problems.Operations on organs in the lower body, such as theprostate, the bladder or large bowel, can also have aneffect on your sexual function.DrugsOver 200 drugs used for other illnesses can affect yoursex life. They include drugs to help depression, worry orhigh blood pressure. This kind of side effect may beonly short term, or sometimes other drugs can befound that won’t cause this kind of problem. In eithercase, it’s important thatthe prescribed drugsare taken, becausethe illnesses theytreat can be veryserious. Also, theillnesses themselves cancause sexual problems.PROBLEM...WHAT PROBLEM?19www.trebest.webs.com
  21. 21. Smokingand alcoholDrinking toomuch alcohol orsmoking a lot of cigarettes can damage the blood vesselsand nerves that affect your ability to have an erection.Giving up any of these can have a positive effect on yoursex life — if the habit is stopped in time. It may take awhileforyourbodytorecoverfromthe use of alcohol, drugs orcigarettes. Your doctor can giveyou good advice about what youmight expect.Emotional problemsEmotional problems can play a major role in beingable to get and keep an erection, and they are oftenpoorly understood.PROBLEM...WHAT PROBLEM?20www.trebest.webs.com
  22. 22. One of the reasons most often mentioned as causingsexual problems is stress. Few people today are totallyfree of stress or worry. The stresses in our world, both athome and at work, have become part of our lifestyle.Because stress lowers the levels of important male bodychemicals, a man might not want to have sex as often, and there might be a drop in the ability to perform whenhe does try. Some of the other emotional issues that may causesexual problems are:q thinking too muchabout other issuesand forgettingabout yoursex lifeq shynessq concern aboutperformance orabout the size ofyour penisq being ina newrelationshipq worry about along-term relationship.PROBLEM...WHAT PROBLEM?21www.trebest.webs.com
  23. 23. Another big issue is fear of failure – it’s hard to believethat fear of failure causes failure, especially when you areso determined to succeed.Other emotions such as depression, worry or anger canalso affect your sex life. Sometimes these feelings are sodeep that you are not even aware of them and the effectthey are having. This is when a doctor might suggestgoing to a counselor (a person who works withemotional problems). These experts can give advice onwhy you have a problem and then help to solve it.Remember! At all ages, your desire to have sex and your ability to getand keep an erection is affected by:q your overall healthq what happens in your social lifeq how happy or positive youare about life in generalq what sort of attitude you haveto problems not related to sexq your lifestyle — how busy you are, what else is on your mind at the time. Also, remember that for most men who cannot get orkeep an erection the problems can be treated. A visit tothe doctor can usually sort out a treatment that works.PROBLEM...WHAT PROBLEM?22www.trebest.webs.com
  24. 24. Having a problem getting or keeping an erection is a bigdeal for a man. You might still feel like you want to havesex but you can’t because your erection is not hardenough, or it goes away too quickly or might not last longenough to satisfy your sexual partner.Men don’t often know why their sex lives have started tobe a problem and they can feel sad, frustrated or maybeeven angry. In a relationship, sexual problems like thiscan lead to problems like arguments, blame, lonelinessor sadness. Some men hide their difficulties from their partners.Unfortunately, this doesn’t solve the problem or make itgo away. Sometimes it creates more problems. Yourpartner may feel unloved or unwanted — or might thinkyou are having an affair or want to end the relationship. Not being able to have sex can greatly affect both yourlife and your partner’s.If you are experiencing a problem having sex — gettingan erection or keeping one — there is help availableand there are medical treatments. The first step is to seeyour doctor. If you are having a problem23www.trebest.webs.com
  25. 25. Here’s what some men with sexual problems have said totheir doctors:IF YOU ARE HAVING A PROBLEM24“I kept mythoughts to myselfand becamevery bitter”“Im afraidof losing mypartner...”“I feel lessof a man”“It cant begood for mypartner...”“I began tofeel like it wasall my fault andthat there was nosolution...”“A huge senseof loss and anincreasing feeling ofuselessness”“Im afraidto getmarried...”www.trebest.webs.com
  26. 26. Among the many things thathave kept men fromtalking about their sexualproblems is the fear that“the problem is in the mind”. Hopefully, you nowunderstand that if you are having aproblem getting or keep-ing an erection, it maybe because of a physicalreason that canbe treated.Talk to yourpartner. Andtalk to your doctor. These are someexamples of the questions doctors getasked most often on this page. Youmight like to use them, so youcan find out all you need toknow about your problemand about treatment. What should I say to my doctor ?25Why is myerection not likeit used to be ?Why do I have anerection in the morningbut not at other times ?Why am I havingdifficulty gettingan erection ?Are there anyside effects ?What is normalfor my age ?How muchdo treatmentscost ?www.trebest.webs.com
  27. 27. WHAT SHOULD I SAY TO MY DOCTOR ?26If you go to yourdoctor, it might be agood idea to take a pen along, so you can make notes ofwhat your doctor says. You may alsowant to write down somequestions of your own beforeyou go.Do treatmentsreally work ?Why am I notfeeling as interestedin sex as usual ?www.trebest.webs.com
  28. 28. There are lots of myths about sex and sexual ability. These are a few of the ones heard most often — and thereal answers to the statements...Facts and fiction27Your sex life cancarry on well into oldage. Younger men may bemore healthy and energetic sothey feel like having sex muchmore often — but this has noeffect on what they will be able todo when they are much older. Many older men (in their70s, 80s and more) havebecome fathers.“Too much sex when you’reyoung will mean you can’t do itas much when you are older”WRONGSomepeniseshave a slightcurve. This iscompletelynormal.WRONG“An erectpenis must beabsolutelystraight”Germs canenter the bodythrough the skin ofpenis and this can happenin a few seconds. Trying towash out any germs after sexcan be too late. Wearing acondom is the best way toreduce risk to you oryour partner.“Urinating after sex willwash out the possibilityof an infection”WRONGwww.trebest.webs.com
  29. 29. FACTS AND FICTION28“Men who can have more thanone erection during sex are moremasculine than men who canonly have one erection”WRONGAll men need arest after ejaculationwhile the body replacessemen. The time for restingusually gets longer with age as yourbody’s overall function slows down. Also, some men might have only oneerection during sex and then theycontinue the intimacy of sex inother ways. Sex is based onshared experience, not asingle performance.Lastinglonger is aninternal sense,an awareness and afeeling of controland it can belearnt.WRONG“Men withcircumcised peniseslast longer duringsexual intercourse, because the headof the penis isless sensitive”“It’s all in the mind, there is no real problem”WRONGAn erectionproblem has aphysical cause. Usuallyit can be a sign of anotherillness or can occur becausesome medicines haveside effects thatcause this typeof problem.www.trebest.webs.com
  30. 30. FACTS AND FICTION29“If a man can’t keep an erection it meansthat he doesn’t love his partner any more”WRONGYou maybe so involved withyour partner that you try toohard to have an erection. Becauseemotions can be so intense, you canfeel under stress and the stress itself canstop an erection. These feelings canbecome a downward spiral of failing tohave an erection and then feelinginadequate. No matter how hard you maytry to relax or whatever other approach isused, the spiral of trying and failingmay continue. This is when outsidehelp should be looked for and avisit to the doctor couldreally help.It canalmost always betreated. Millions ofmen are enjoyingsex again aftertreatment.“Once you have aproblem witherection you justhave to learn tolive with it”WRONG“Only old menare affected bythis condition”WRONGMen of allages are affected.By the age of 50almost all men will haveexperienced someproblems in havingand keeping anerection.www.trebest.webs.com
  31. 31. FACTS AND FICTION30Erectionproblems usuallyinvolve other healthproblems which can include:q problems with blood flow in thebody (such as high blood pressure)q diabetes (changes in bloodsugar levels) or its treatmentq shock or woundsfrom accidents oroperations.“Not being able to have anerection is a sign of a loss ofmanhood or lack of sex drive”WRONG“Only a few men sufferfrom problems like this”WRONGOver 100million men (1 inevery 10) in the worldare not able to have anerection at some time intheir lives — it is avery commoncondition.“Women prefer men withlarge penises so that theywill have an orgasm”WRONGThe nerves thattrigger a woman’sorgasm are near theentrance to the vagina sopenis size does not reallymatter. Often it is reallyother factors in therelationship thatencourage sexualresponse.www.trebest.webs.com
