  2. 2. ▶ A third level qualification in a health/social sciences/business/management subject plus two years project co-ordination experience in health, education, youth or community development Or ▶ Three years project management/co-ordination experience in health, education, youth or community development. Experience of creating and implementing personalised, outcome-focused plans and risk assessments for individuals. Qualifications
  3. 3. There is no ‘one size fits all’ model of care for Social Prescribing. The level of support can vary from individual to individual. However, there are certain skills that are used continuously.
  4. 4. ● Active Listening ● Empathy & Validation ● Using Good Questioning Skills ● Motivational Interviewing ● The Ability to Put Someone At Ease & Gain Their Trust ● Checking & Understanding ● Summarising COMMUNICATION SKILLS ARE KEY
  5. 5. ● Excellent leadership and facilitation skills ● Excellent communication skills ● Excellent report writing skills ● Excellent organisation and time management skills ● Excellent use of database programmes ● Ability to use computerised systems SPECIAL APTITUDES
  6. 6. ● Mental Health First Aid ● Lone Worker & Vulnerable Adult ● Safe Talk ● ASSIST ● Data Protection ● Everyday First Aid REQUIRED TRAINING
