Dell SC9000 and SC4020 All Flash Solutions for MEDITECH Page 5 Overview The rapid, unpredictable growth of data is forcing healthcare providers to consider how their data storage decisions affect patient care. Relying upon paper patient records raises the chance of medical errors, and the increase of data-intensive image archiving has made predicting future storage needs difficult. Healthcare is a 24/7 operation. System outages are unacceptable and slow access to records impacts the speed at which doctors can treat patients. Plus, there are long-term storage needs to consider; data has to be rapidly available throughout the life of the patient in order for hospitals to comply with government regulations. To date, traditional IT storage solutions have added costs and complexities instead of reducing them, effectively defeating the advances technology is intended to provide. To address these issues and provide better patient care, IT departments are moving beyond mere storage management and embracing the all-encompassing approach of Dell Healthcare Storage Solutions. This whitepaper focuses on Dell’s storage solutions for MEDITECH. Storage is a critical component of any MEDITECH solution. With Dell, our Dell SC storage arrays are an integral part of our meticulously crafted end-to-end MEDITECH solutions. These enterprise-class solutions are a combination of tightly integrated and fully tested systems components, software and MEDITECH-specific services – a complete solution for hospitals. They are the result of years of extensive collaboration between Dell, BridgeHead Software and MEDITECH and are consistent with Dell’s philosophy of “Open, Capable and Affordable.” Dell is pleased to offer hospitals these contemporary MEDITECH solutions that address the drawbacks and limitations that are associated with traditional legacy storage solutions. Dell SC4020 and SC9000 All Flash MEDITECH Solutions The Dell SC4020 and SC9000 All Flash Array MEDITECH Solutions are fully integrated combinations of Dell system components, BridgeHead Software and Dell services. The Dell end-to-end configurations are shown in Figure 1 below. The Dell SC All Flash Solution that is appropriate for your hospital depends on your hospital size, workload requirements and budget. A skilled Dell MEDITECH Solutions Architect can help you determine the solution that is best suited to your particular environment. The components inside the dashed line box are typically deployed in the hospital’s data center for an on premise solution.

The servers are connected to the Compellent SC All Flash Storage Center via dual- redundant fibre fabrics using Brocade switches. Hospital administrators access the MEDITECH server applications from MEDITECH client applications running on Windows-based desktop PCs via standard TCP/IP networking. Dell Networking switches provide the inter-connection points for this network. While the Dell Networking switches offers numerous benefits, our Dell MEDITECH solution will also integrate with a hospital’s existing enterprise- class IP switching infrastructure. BridgeHead ISB/IDR can be configured to use a wide variety of industry standard backup target devices. The world’s largest companies could achieve massive business results while simultaneously enjoying the lowest TCO-per-GB of storage. Today, your hospital can access these powerful techniques starting with a compact, affordable, yet feature-rich and scalable solution from Dell, the Dell Compellent SC4020 and SC900 All Flash Arrays (AFAs). No Fork Lift Upgrades The Dell Compellent Fluid Data architecture was designed to overcome the inefficiencies found in traditional storage systems. Traditional monolithic storage architectures are physically bound architectures with virtualization added to them as a “bolt on” feature to their 20+ year old architectures. Some of the basic limitations of a traditional storage system are as follows:  Forklift upgrades with forced data migrations that must be executed approximately every 3 to 5 years  Costly data-in-place upgrades between different models within the same storage system family of products  Software licenses tied to the frame that must be repurchased when performing tech refreshes  Lack of true Storage Virtualization which creates limitations on pooling, decreasing flexibility and increasing system administration costs  Limited Tiering capabilities such as tiering by disk drive types only, excluding RAID level tiering offered by Dell Compellent  Limited Snapshot capabilities, support for a limited number of recovery points per volume and per system with noted overhead for pre-allocation of space  Limited Thin Provisioning – thin provisioned volumes must be predetermined and managed separately with noted performance implications (usually not recommended for high performance data volumes)  Complicated Replication Strategies – replication configuration is usually complicated and often requires additional hardware and software external to the storage system adding cost and complexity  Complicated Data Mobility – online volume migration is only supported with systems that require additional hardware and complexity  Complicated storage management and limited storage reporting capabilities  Firmware must be upgraded on multiple components –the firmware on the multiple components must be kept at supported levels between them, even if all of the component are not utilized  Relies heavily on cache for performance – most traditional storage systems have a very large front-end cache that is necessary because of the way they implement physical RAID Groups and drive pools The Dell Compellent Fluid Data Architecture was designed as a next-generation storage system built from the ground up to enable true virtualized storage, automation, and efficiency that cannot be found in the traditional storage architectures. As the data ages, it is automatically identified and moved to less expensive storage until it becomes more active again. Dell’s fully virtualized Storage Center platform enables unprecedented levels of efficiency and automation. All available storage is combined in a single pool by default — and since individual disks and volumes can support multiple RAID levels, pre-allocation of RAID groups is not required. The performance and capacity of the entire pool is presented to all RAID levels simultaneously, which dynamically expand or contract as needed based on real-time usage metadata tracked at an extremely granular 512KB block level. Dell SC9000 and SC4020 All Flash Solutions for MEDITECH Page 11 BridgeHead Software MEDITECH ISB/IDR A major component of the Dell Compellent MEDITECH solution is the fully integrated BridgeHead ISB/IDR software. The ability to back up and restore a healthcare information system (HIS) is critical for hospitals should they be subject to a disaster, system outage, loss or corruption. To make a transactionally consistent backup of a complex database application, such as MEDITECH’s Healthcare Information System, requires full co-operation between the application and the backup tool. If this is not the case, the backups are likely to be compromised with partially updated and/or inconsistent data. BridgeHead Software and MEDITECH co-designed the Integrated Serverless Backup (ISB) API in 2002 to allow both the backup and HIS application to work together to produce a tried and tested backup solution. BridgeHead was not only the first partner to use this approach, but remains the only vendor that supports all MEDITECH certified storage arrays. BridgeHead Software, Dell Compellent and MEDITECH collaborated and worked closely in the development and integration with the MEDITECH Backup Facilitator (MBF), a key integration technology for automating application consistent backups of MEDITECH applications. Technical Overview MEDITECH applications demand high performance from storage arrays. But backup, by its nature, reads all the data which, if performed on a live MEDITECH server, would dramatically impede performance. One of the key features of the ISB Agent is that the backup reads the data on the backup server not the MEDITECH server. The Dell Compellent storage array is used to create a consistent point in time copy of the data using the Dell Compellent Data Instant Replay feature. This copy is then presented to the backup server, removing all I/O overhead from the MEDITECH servers (generally, one backup server can service around 25 MEDITECH servers). The same technology can be leveraged to make a point in time copy within the Dell Compellent storage array. However, instead of backing up a copy to tape (or disk or VTL etc.), it can simply be kept on the storage array for fast restore. Integrated Disaster Recovery (IDR) allows speedy operational recovery with no delay in terms of getting a tape back from an external store and no waiting for one restore to complete before the next can start. Multiple generations of consistent point in time copies can be preserved without any impact on the MEDITECH system. BridgeHead Software also provides the widest range of IDR methods by leveraging the power of a variety of storage arrays to provide the best operational recovery options. Depending on the particular storage array these range from the most basic ‘copy the data to another disk’, through the creation of multiple generations of space efficient snapshots to the creation of replicas on secondary arrays in secondary data centers (with options to rapidly switch to live running in the secondary location). In 2012, after over 10 successful years of the ISB API, MEDITECH and BridgeHead Software have designed the next generation of system protection MEDITECH Backup Facility (MBF) based ISB and IDR. It is available today with Dell Compellent. MBF retains all of the functionality of the ISB API, but fine tunes the balance of work so that the Dell Compellent array manipulation is solely performed by the backup application. This allows more advanced features such as Consistency Groups to be used to reduce the ‘pause time’ while a backup is initiated. Ultimately, MBF enables even quicker production of backup copies with reduced impact on MEDITECH users.

For example, in a VMware environment, RDM (Raw Device Mapping - which can be thinly allocated on the array) is used to take advantage of the Dell Compellent storage array hardware based virtualization rather than using VMware’s software virtualization which eats processing power from the ESX server and MEDITECH VMs. BridgeHead supports many different IDR methods using facilities such as thinly allocated snapshots to create multiple generations of copies at minimal storage consumption and I/O load. Block Level Backup and Restore for Speed While the emphasis for the MEDITECH OSAL Database or Client/Server File Servers is on the database consistency requirement, for EMR or SCA the biggest obstacle to backup is the millions of small files. Any file oriented backup becomes unworkably slow (and restore even worse) simply because just opening and closing all the files takes so long. This is where the block oriented side of ISB comes to the fore. Because ISB is working on a frozen point in time copy of the entire volume the backup can be a block mode copy of the underlying disk. This operates at device speed with no overhead caused by the number of files. A typical ISB backup runs at between 20 and 100 MB/sec depending on the storage array and destination media.
