  2. 2. Pre-injury StatusBefore his injury this 21 year oldman was a manager of a distributionwarehouse. He worked in excess ofthe required full 40 hours.He was in the army before and wasin the reserve. He lived with hisfather, whom he helped andsupported.He dated and had many friends withfrequent social contacts. He drove aso-called muscle car, which wasactually wrecked in the accident. Hehad no pre-existing conditions andwas perfectly healthy.
  3. 3. Present ConditionAfter his injury this patient has notresumed working. He is at homeon long term disability leave.He has taken up social activitiesand they are at a level of at leastone half of that before the injury.He is completely independent athome and can be left alone for 8hours without a problem.He does not drive at this time, butis able to take the bus or call ataxi, and has gone out on his ownto shop.
  4. 4. Present ConditionThere are no complaints of anypost-injury symptoms and hedoes not report any strain ordisruption of his normal life. Hereiterates that he would love tobe back at work.
