Pre-injury StatusBefore his injury, this patient was35-year-old construction worker witha steady job in commercial building.He was married and had twochildren, ages 11 and 15.He went out regularly with his wifeand work friends. His favorite hobbywas riding his quad all-terrain vehicle(ATV), which he did on weekends.There were some concerns aboutalcohol use, especially during theweekend outings.

Present ConditionAnd three months prior to his injuryhe broke his arm while driving hisATV, possibly after a number ofbeers.After his injury, this patient has beeninstitutionalized since the transferfrom the hospital.He is completely paralyzed except forhis ability to move his right thumband forefinger. He has a permanenttracheostomy and cannot talk.
  4. 4. Present ConditionHe has to be fed by a tube and hasvery limited means of communicationwith the outside world.He responds to pain and can interactwith his wife and oldest child byresponding to their questions withfinger movements. He will lift hisfinger and thumb when asked to.
