CRISIS MANAGEMENT PLAN FOR NAME OF NUS STUDENTS’ / STAFF’S EVENT / ACTIVITY (LOCATION/VENUE) (EVENT DATE/PERIOD) Instruction In Completing This Plan Replace the (Name ) with the name of the appointment holder. Replace the ORGANISATION with the organisation organising the event / activity. Primary Responsibilities 1 Project Director (Name 1) - Provides the overall coordination to the group’s emergency response operations. 2 Deputy Project Director (Name 2) – Assists Project Director (Name 1) in managing the crisis or emergency. 3 Supervising Officer/Advisor of the ORGANISATION (Name 3) - Approves the plans and safety coverage of the activities/event as proposed by the Project Director (Name 1). Provides supervision and guidance to Project Director (Name 1) and Deputy Director (Name 2) throughout the conduct of event. 4 Deputy Project Director (Name 2) and ORGANISATION representative (Name 4) - Assists Project Director (Name 1) in managing the crisis or emergency in Singapore. To ensure the availability of ORGANISATION Conference/Training Room or appropriate venue for Press Conference. To secure the availability of NOK Holding Area at ORGANISATION club house/lounge/training/conference room or appropriate venue in times of need. 5 OCR (Leila Thayalan/ Mabel Wong) – Manages communication and relations with the media and press. Manages the official NUS official message to the various stakeholders. 6 Hotline Manning - Deputy Project Director (Name 2) to provide and co-ordinate for the availability of NUS Hotline Manning Staff and XXXXX Pte Ltd to provide staff (Names) to man Crisis Hotline and other administrative support. 7 NUS Counselling Centre – Provides professional counselling support. 8 CARE Network Members of OSA and ORGANISATION (Names) - Provide emotional support to affected group member and/or their next-of-kin. 9 Safety Officer (Name 5) - Acts as the Point-of-Contact between Supervising Officer/Advisor of the ORGANISATION (Name 3), Project Director (Name 1), Deputy Project Director (Name 2) and the Executing Team/Group on site or in overseas. Coordinates the overall response operations on site or from overseas. 10 Group Leader (Name 6) assisted by XXXXX Pte Ltd Representative (Names) and Safety Officer (Name 5) - Acts as the administrative and logistic coordinator for the Team/Group on site or in overseas. Also serves as the Point-of-Contact between Supervising Officer/Advisor of the ORGANISATION (Name 3), Project Director (Name 1) and the Team/Group. OSHE CAA 161106 1

Informing the Supervising Officer/Advisor of the ORGANISATION (Name 3), Project Director (Name 1) and Deputy Project Director (Name 2) and CEM Director (Dr Peck Thian Guan) Group Leader (Name 6) assisted by XXXXX Pte Ltd Representative, (Names) and Safety Officer (Name 5) shall inform the Supervising Officer/Advisor of the ORGANISATION (Name 3), Project Director (Name 1), Deputy Project Director (Name 2) and CEM Director (Dr Peck Thian Guan) of the incident immediately, depending on the severity of incident. 2. Informing President (Prof Shih Choon Fong) When CEM Director (Dr Peck Thian Guan) is notified of any incident or accident by Group Leader (Name 6), Project Director (Name 1) and/or Supervising Officer/Advisor of the ORGANISATION (Name 3), CEM Director (Dr Peck Thian Guan) is to inform NUS President (Prof Shih Choon Fong) of the incident immediately. 3. Informing CEM Team The CEM Director (Dr Peck Thian Guan) shall in turn notify the relevant members of CEM Team and Crisis Executive Group (CEG). 4. Informing Next-of-Kin a. In case of injury that is life threatening or death – President (Prof Shih Choon Fong) or his representative (Deputy President or Vice President) shall inform the Next of Kin (NOK). b. In case of serious injuries – The Supervising Officer/Advisor of the ORGANISATION (Name 3) and the Project Director (Name 1) shall inform the NOK. c. NOK data is provided by Deputy Project Director (Name 2). d. Where possible, NUS President (Prof Shih Choon Fong) or his designate shall meet the NOK and offer a word of encouragement. 5. Assigning CARE Network Members from the OSA, the ORGANISATION and from XXXXX Pte Ltd OSA and the ORGANISATION are to assign at least 2 staff &/or student leaders and 2 staff from XXXXX Pte Ltd as Care Network Members. Duty of Care Network Members is to look after the needs of NOK. Care Network Members will accompany NOK to incident site. 6. Travel to Incident Site In the event of S/No 4a, the Supervising Officer/Advisor of the ORGANISATION (Name 3) and Project Director (Name 1), with the assigned Care Network members (Names), shall accompany the NOK to the Incident Site and make contact with the Group Leader (Name 6) and Safety Officer (Name 5). The Supervising Officer/Advisor of the ORGANISATION (Name 3) assisted by the Project Director (Name 1) shall command the incident from Country/Site. The Deputy Project Director (Name 2), assisted by ORGANISATION (Names) shall remain in Singapore and liaise with agencies here and with the Group in Country/Site. 7. Managing Crisis On Site Group Leader (Name 6) assisted by Safety Officer (Name 5) and XXXXX Pte Ltd Representative, (Names) shall be responsible for managing the incident on site. He/She shall liaise with the Supervising Officer/Advisor of the ORGANISATION (Name 3) and the Project Director (Name 1). They shall hand over the command and control to the Supervising Officer/Advisor of the ORGANISATION (Name 3) upon the completion of the briefing and update of the latest situation and development on site.   3. 3. 8. Setting up of Emergency Operation Centre (EOC) Senior Manager, Campus Security, OED (P Samynathan), shall set up the EOC upon activation by CEM Director (Dr Peck Thian Guan). The EOC will be manned by OSHE EM Team (Yam Guan Shyh/Louise Ng). 9. Setting up of Media Room The Media Room shall be at the ORGANISATION Conference/Training Room or appropriate venue. The Deputy Project Director (Name 2) and ORGANISATION representative (Name 4) are responsible for its availability and its proper setup for Press Conferences. 10.Setting of NOK Holding Area The Deputy Project Director (Name 2) and ORGANISATION representative (Name 4) shall set up a NOK holding area in the ORGANISATION club house/lounge/training/conference room or appropriate venue. 11. Manning of Crisis Hotline and Website In an event of a crisis, OCR shall set up a Crisis Website where latest information about the crisis will be posted. Three Crisis Hotlines will be set up in the EOC and will be manned by NUS Hotline Manning staff co-ordinated by OSHE EM Team (Yam Guan Shyh/Jeannie Koh/Louise Ng). OSHE EM Team (Yam Guan Shyh/Jeannie Koh/Louise Ng) shall be responsible for identifying these staff and set up the duty roster. Staff is expected to man the hotlines 24 hrs during the initial stages of the crisis. An OCR manager (Leila Thayalan) shall be contactable to provide advice to hotline crew should they encounter questions beyond their scope.
