Overview Goal:  Enhancing quality and efficiency of medical  homes through the use of :• Coordinated care management networks• Regional support for medical home maturation• Regional medical and pharmacy directors• Regional medical management meetings• Non punitive provider comparisons• Provider / medical director engagement in  Agency policy development 2

Approach• Lessons learned from North Carolina via Alabama visit  from expert, extensive reading, and site visit to North  Carolina• Broad based work group to develop our plan• Road shows and webinars to inform / stimulate  discussion • Identification of local champions• Funding based on budgeted shared savings from  existing medical homes• 3 initial pilots with 3 other areas already requesting  networks 3

Lessons Learned / Challenges• Dollars although modest are not insignificant:   Allow room in your budget for the unexpected• Outreach phase needs to be provided generous  time:  It takes time just to have providers begin  to understand what you are trying to do• Request for proposals (RFP) development is a  team project:  This is a wide reaching  development and a lot of perspectives are  required• North Carolina and NASHP have been very open  with their time to help us 4
