Tip: You will also receive a confirmation e-mail with the same bar code. Your bar code can be scanned directly from your smart phone or print out on the day of your appointment.

A FEW REMINDERS
Be prepared to pay the required fees on the day of your appointment.
You must have your confirmation barcode with you on the day of your appointment or you will be treated as a “walk-in”.
Don’t forget to have all your completed forms and applications with you on the day of your appointment.
