• Petition for Appointment of Conservator
• Confidential: This form provides confidential data to the Court
• Capacity Declaration: A doctor has to evaluate the Proposed
Conservatee
• Conservatorship Court Hearing
• Order is signed by the Conservatorship Judge
• Letters: This is the authorization to act as Conservator
• Recording the Letters with the County Recorder
• Level of Care – Due 60 days from Appointment
• Inventory and Appraisal – Due 90 days from Appointment
