Navigate to the item that you would like to order and add to
your shopping cart.

Navigate to the “Additional Shipping Address” product and
add it to your shopping cart.

In the upper right-hand corner of your screen, click on your
shopping cart.

In the “INSTRUCTIONS” box below the “Additional Address”
item, type in: the name of the product and the address.
