To get Rank S+, the highest in the game, you can only do it from a game
starting from scratch; no playing New Game Plus. In other words: Capcom
doesn't want the experience of getting S+ to be "cheatable" using tricks like
extra weapons such as the rocket launcher with infinite ammo.

different difficulty modes and meeting different requirements:
Knitted Cap, Iron Helmet, Glasses (Square) - Beat the game on
Standard Mode with an A rank.
Deer antlers - Complete the game on Standard Mode with an S+ rank.
Glasses (round), Flight Helmet, Surgical Mask - Beat the game on
Knight Armor Outfit (Ashley) - Beat the game on Hardcore Mode with
Chicken Head (Leon) - Beat the game on Hardcore Mode with Rank S+.
Hunk Helmet - Complete the game on Professional Mode.
Handcannon - Complete the game on Professional Mode without using
any extra weapons. It cannot be done in New Game Plus; it has to be in
a New Game.
Chicago Sweeper - Complete the game on Professional Mode with
Cat Ears - Complete the game on Professional
Mode with an S+ rank.
In summary, if you want to get the highest rank possible in
Resident Evil 4 remake (S+), it's time to start a new game from scratch and
complete the game very quickly, which will force us to be very clear in
