  4. 4. SBAR PROCESS <ul><li>(S) SITUATION: What is the situation you are discussing with your colleague or mentor? </li></ul><ul><li>Identify yourself, patient, unit, room number </li></ul><ul><li>Briefly state details of the patients issue </li></ul>

SBAR cont. <ul><li>( B) Background: Pertinent background information related to the situation could include the following: </li></ul><ul><li>- The admitting diagnosis and date of admission. </li></ul><ul><li>- List of current medications, allergies, IV fluids, and labs. </li></ul><ul><li>- Most recent vital signs </li></ul><ul><li>- Lab results: provide the date, time and results of any previous tests for comparison. </li></ul><ul><li>- Other clinical information </li></ul><ul><li>- Code status (DNR) </li></ul>
  6. 6. SBAR cont. <ul><li>(A) Assessment: Present your assessment of the situation. </li></ul><ul><li>(R) Recommendation: State your recommendation or the patient’s request. </li></ul><ul><li>- Examples: </li></ul><ul><li>Patient needs to be seen now. </li></ul><ul><li>Order change. </li></ul>
