These educators then advised HCPs regarding medication management for their patients with diabetes — in consultation with the endocrinologists. * As you can see from the chart below, the Diabetes Ambassadors Program helped lower patients’ A1C by 1 percent. The Diabetes Ambassadors program is also being implemented via pharmacists at other health plans.
