MR parameters divulge depth of acute hamstring injuries
Professional athletes and dancers are as familiar with hamstring strains as they are with the
bench press and barre, respectively. Two studies affirmed MR's major role in defining these
injuries. Australian and British researchers used the modality on football players for reinjury risk
stratification, while Swedish sports medicine specialists looked at hamstring strains in dancers
during slow-speed stretching. While the MR results didn't hold all the answers, the imaging exam
was still worth performing, according to the experts.
Germany worried about excess use of CT scans
BERLIN (Reuters), Jul 12 - Germans with no real health problems are needlessly taking in heavy
doses of radiation because of an alarming increase in the use of computerized body scanning,
the country's environment minister said. ording to a study by Deutsche Bank published last year.
Germany's Federal Office for Radiation Protection wants a critical study of the situation and a halt
to unnecessary CT scans, said German Environment Minister Sigmar Gabriel.
PET scans useful in identifying colorectal cancer recurrence
NEW YORK (Reuters Health), Jul 9 - Positron emission tomography using 18-fluoro-2-deoxy-
glucose (FDG-PET) can identify colorectal cancer recurrence in patients with suspicious clinical
or radiological findings but normal carcinoembryonic antigen (CEA) levels, according to a report
in the June 7th issue of the World Journal of Surgical Oncology.
MR shows broad spectrum of findings in pregnant women with suspected appendicitis
Although acute appendicitis is not terribly common in pregnant women, it does pose a diagnostic
challenge because of anatomic and histologic changes that occur in these patients. CT carries a
radiation risk while ultrasound, which is the established modality in this population, doesn't always
deliver the whole story. Instead, Boston-based imagers have found success with MRI for
evaluating the appendix, and more, in the gravid patient.
Colonoscopy more often successful in mornings
NEW YORK (Reuters Health), Jun 22 - Colonoscopy is more likely to be completed in its entirety
when it's performed in the morning than in the afternoon, a Mayo Clinic study shows. The
difference in success rates is most likely due differences in the adequacy of bowel cleansing
before the procedure.
Patient survey suggests preference for minimal-prep VC
While colorectal cancer is the second leading cause of death in the U.S., less than half of
screening age individuals over 50 are examined for the disease. A new survey suggests that
eliminating the cathartic bowel preparation might improve compliance screening rates. But the
answers may not predict patients' real interest in screening, and the minimal-prep approach is not
without complications, cautioned a radiologist experienced in virtual colonoscopy screening.

Molecular Imaging of Inflammation in Atherosclerosis With Targeted Ultrasound Detection
of Vascular Cell Adhesion Molecule-1

Background-: The ability to image vascular inflammatory responses may allow early diagnosis
and treatment of atherosclerosis. We hypothesized that molecular imaging of vascular cell
adhesion molecule-1 (VCAM-1) expression with contrast-enhanced ultrasound (CEU) could be
used for this purpose.
Methods and Results-: Attachment of VCAM-1-targeted and control microbubbles to cultured
endothelial cells was assessed in a flow chamber at variable shear stress (0.5 to 12.0
dynes/cm2). Microbubble attachment to aortic plaque was determined by en face microscopy of
the thoracic aorta 10 minutes after intravenous injection in wild-type or apolipoprotein E-deficient
mice on either chow or hypercholesterolemic diet. CEU molecular imaging of the thoracic aorta
10 minutes after intravenous microbubble injection was performed for the same animal groups.
VCAM-1-targeted but not control microbubbles attached to cultured endothelial cells, although
firm attachment at the highest shear rates (8 to 12 dynes/cm2) occurred only in pulsatile flow
conditions. Aortic attachment of microbubbles and targeted CEU signal was very low in control
wild-type mice on chow diet. Aortic attachment of microbubbles and CEU signal for VCAM-1-
targeted microbubbles differed between treatment groups according to extent of VCAM-1-positive
plaque formation (median CEU videointensity, 1.8 [95% CI, 1.2 to 1.7], 3.7 [95% CI, 2.9 to 7.3],
6.8 [95% CI, 3.9 to 7.6], and 11.2 [95% CI, 8.5 to 16.0] for wild-type mice on chow and
hypercholesterolemic diet and for apolipoprotein E-deficient mice on chow and
hypercholesterolemic diet, respectively; P<0.001).
Conclusions-: CEU molecular imaging of VCAM-1 is capable of rapidly quantifying vascular
inflammatory changes that occur in different stages of atherosclerosis. This method may be
potentially useful for early risk stratification according to inflammatory phenotype.

ACL Impingement Prediction Based on MRI Scans of Individual Knees

Although tibial external rotation and abduction do not load the ACL strongly in cadaver-based
biomechanical studies, such knee positions are associated with ACL injuries in clinical practice.
We hypothesized the ACL could be injured in such knee positions because of its impingement
against the intercondylar notch. We developed a three-dimensional geometric ACL impingement
model through segmentation of MR images of individual knees. We investigated impingement by
determining the deformed geometry and elongation of the ligament as it wrapped around the
notch surface during impingement. When impingement did not occur, the gap width separating
the ligament and the notch surface was computed. Tibial external rotation/abduction could cause
the ACL to impinge against the lateral notch wall and elongate as it wraps around the surface of
the notch wall. The impingement occurred between the middle portion of the ligament (at 45% +/-
8% [mean +/- standard deviation] of the ligament length) and the convex surface of the lateral
notch wall. Considering the multiband fiber architecture of the ligament, the anteromedial band of
the ACL sustained greater elongation during impingement and showed a smaller gap width from
the notch surface than the intermediate band or the posterolateral band.
