    2. 2. 2 Telemonitoring as a key enabler Telemonitoring technology enables people to monitor their own clinical signs, health diary and questionnaires from home. The data is synchronized daily to a central repository which is web based and can be accessed by the patient‟s health professionals. Supports and encourages self-management
    3. 3. 3 Background • Established in response to the Ministry of Health‟s call for “Better, Sooner, More Convenient Care” • Te Whiringa Ora was established to provide a consistent regional approach for patients with long term conditions and complex needs – “high users of hospital services” • Home based, community focus • Make better use of existing resources (provider ecosystem) • Greater use of supervised but unregulated staff • Greater use of patients own personal resources
