  2. 2. Background  Approximately 10% of the population have Chronic Kidney Disease (CKD)  Early detection and management is better and more cost effective  The majority of CKD patients can be managed in primary care  Secondary care needs to see the complex patients and support primary care
  3. 3. The Challenge  Identifying and knowledge of patients  DHB audit shows it knows only 1/3 of patients with mod/severe kidney disease  Risk stratifying patients  Managing resources  Creating timely systems and processes  Improving integration and collaboration
  4. 4. Progress to Date  Collaboration with Ministry of Health  Development of National Consensus Statement.  Development of a CKD Working Party  Linkage of CKD Pilots including IT system  Discussion underway on HSAGlobal + DHB innovation partnership to prove model
  5. 5. Future Goals  Endorsement of CKD Consensus Statement  Further development and dissemination of CKD IT tools  Better management of CKD between primary and secondary care
