(Sudanese Chest Sonography Workshop (Pleural ultrasound)

  1. 1. Pleural UlrasoundGamal Rabie Agmy ,MD ,FCCPProfessor of Chest Diseases, Assiut UniversityERS National Delegate of Egypt
  2. 2. • Immediate bedside availability• Immediate bedside repeatability• Rapid goal directed application• Cost saving• Reduction in radiation exposureAdvantages of TransthoracicUltrasonography
  3. 3. Scanning Positions forChest Sonography
  4. 4. 1-Frequently very high in the supinecritically ill patient2-Particular caution in post CABGpatients -unilateral diaphragmaticdysfunction may confuse the examiner3-Massive edema and obesity maydegrade image qualityDiaphragm
  5. 5. *The amount of pleural fluid can beestimated by the following formula:V (ml)= 20 x SepV = volume, Sep = maximal distancebetween the two pleura layers.*For sitting patients a good method is tocalculate the sum of the basal lung todiaphragm distance and the lateral heightof the effusion and to multiply the sum by70.Quantification of Pleural Fluid:
  6. 6. Sonographic signs of pleural fluid1. Echo-free zone separating the visceral andparietal pleura2. Echo-free zone displaying a change of formduring breathing3. Floating and moving echogenic particles4. Moving septations within the pleural space5. Moving lung within the fluid6. “Fluid color” sign – on Doppler sonography
  7. 7. the "seashore sign" (Fig.3).
  8. 8. Absent lung slidingExaggerated horizontal artifactsLoss of comet-tail artifactsBroadening of the pleural line to a bandLung pointLoss of lung impulseThe key sonographic signs ofPneumothorax
