The presentation describes an adaptive approach to explaining the broadest spectrum of pharmacological activity of a course of systemic ozone therapy. This approach is based on the idea of systemic ozone therapy as a form of activation therapy. The difference between ozone therapy and other types of activation therapy is that ozone fixes a specific conditioned-unconditioned reflex of the adaptation system   for the pharmacological activity of ozone.

This approach was the result of attempts to find an answer to the two questions: • First: Why during the course of ozone therapy the patient's condition periodically improves and worsens and • Second question: Why does the therapeutic effect of ozone therapy if achieved persist for 3 to 6 months? • The modern theory of ozone therapy can not explain these features of ozone. Let us consider two examples.

This slide represents a successful course of therapy because for 6 months the patient enjoys the result of treatment.

The course failed because the patient received only a minor and short-term improvement at his health. • What is the reason for achieving the different effectiveness from the standard course of systemic ozone therapy and how do we make the course results predictable? To study this we use the generally accepted quality of life questionnaire Medical Outcomes Study-Short Form, SF-36.
  7. 7. Health related quality of life was studied in random sample of 2156 men and 2769 women aged 45-69 years, according with SF-36 is an indicator of overall health status. This led us to the idea of assessing the subjective age of the patient during treatment. In order to the this we use the results of a statistical analysis of the dependence of the quality of life on age. We chose the parameter of physical role functioning (PRF).

Then throughout the 18 weeks after therapy the achieved value of the studied parameter is preserved. After then spontaneous worsening of the patients' condition takes place.

In this graph we can distinguish three periods: 1) fluctuations in the adaptive age during the course of procedures, 2) the period of adaptive rejuvenation, and 3) the period of development of the ozone withdrawal syndrome

  14. 14. Slide 7 • The duration of remission caused by the course of ozone therapy depends on the age of the patient. The duration of remission is about 6 months at 50 years and about 3 months at 80 years.

  16. 16. Slide 8 • We use the term adaptive rejuvenation in order to emphasize the generalized nature of the processes in the patients' body. Depending on the nature of the underlying disease of the patient we observe a reduction in blood pressure for hypertensive patients or a decrease in glucose concentration for diabetics and so on. It is obvious that we are dealing with the effect of ozone on some integrating system of the body. The real candidate for this role is the hormonal system. This is evident from the dramatic dynamics of the level of cortisol during the course of ozone therapy.

It can be seen that the course of ozone therapy initiates abrupt changes in the level of cortisol in all patients. One month after the completion of the course the level of the hormone stabilizes at a new level. The concentration of cortisol in the blood returns to the baseline level for several months.

  20. 20. Slide 10 • The following slide shows the dynamics of cortisol in the blood in a more detailed form. The arrows mark the end of the course of AHT- O3 procedures. It can be seen that the influence of the course of procedures on the concentration of the hormone is highly individual. The micro- quantity of ozone introduced into the blood causes a pronounced change in the hormonal status of the patient's body. How does ozone affect the level of the hormone and how does the body maintain a new hormonal status for several months?

Ozone as a physiotherapeutic agent According to the widely used terminology, study of natural or preformed natural factors (for example: mud, mineral waters as well as electric and magnetic fields and so on) is a subject of a separate section of medicine the - physiotherapy. From this point of view, ozone is the preformed form of the natural factor - oxygen. Slide 11
  22. 22. Slide 11 • In order to give an answer to this question we must think about the existing theoretical basis of physiotherapy. Why physiotherapy? Because this medical specialty is focused on the study of natural and preformed natural factors. Ozone is a preformed natural oxygen factor. On this basis in a number of countries ozone therapy is accepted as a kind of physiotherapy.

In accordance with the theoretical views of physiotherapy, ozone, like other natural factors, causes a systemic compensatory-adaptive reaction in the body, the basis of which is a conditioned-unconditioned reflex with its neurohumoral component. As can be seen from the illustration, the structures of the intermediate brain and brain stem play the leading role in the organization of the compensatory- adaptive reaction. The most important of these is the hypothalamic-pituitary complex integrating the influence of external and internal factors on the organism and forming an adaptive response. Novelty factor Slide 12

First we develop a conditioned reflex to ozone during ozone therapy and then the role of a reinforcing factor is played by the products of ozonolysis of the blood. Over time the conditioned reflex decays and a new course of ozone therapy is needed to resume it. Physiotherapy teaches us also that the formation of a conditioned reflex occurs in the hypothalamic-pituitary complex. Thus the course of systemic ozonotherapy can be considered completed if the hypothalamic- pituitary complex stops the formation of a conditioned reflex to ozone.

The central element of this system is the hypothalamic-pituitary complex which through the feedback system controls the activity of the adrenal thyroid and gonads glands. In the posterior and anterior part of the hypothalamus there are nuclei regulating the activity of the sympathetic and parasympathetic nervous system respectively.

The data collection time is only 5 minutes and can be performed during the AHT- O3 procedure. O3Test is integrated with O3Navigator.

  56. 56. conclusions 1) Ozone is an preforming natural factor and in accordance with the theoretical basis of physiotherapy is the factor that causes the conditioned-unconditioned adaptive reflex. This reflex decays for several months and requires a repeated course of ozonotherapy for resumption. 2) Fluctuations in the state of health of the patient during the course of ozone therapy are associated with the restructuring of the hormonal system. 3) The restructuring of the hormonal system is normotropic and leads to an adaptive rejuvenation of the patient. 4) The duration of the compensatory-adaptive reaction is determined by the duration of the conditioned reflex to ozone and decreases with age from 6 months in 50 years to 3 months in 80 years. 5) For a successful course of systemic ozone therapy, constant monitoring of the status of the patient's adaptation system should be carried out. Criterion for the completion of the course of procedures is a sharp decrease in the patient's adaptable age.
