• “Metric that helps you understand how you are doing against

5. How to Select KPIs
According to the Health Information and Quality Authority:
• Validity: does the KPI measure what it is supposed to measure?
• Reliability: does the KPI provide a consistent measure?
• Acceptability: are the KPIs acceptable?
• Feasibility: is it possible to collect the required data and is it worth the
resources?
• Sensitivity: are small changes reflected in the results?
• Specificity: does the KPI capture changes that occur in the service for
which the measure is intended?

• Relevance: what useful decisions can be made from the KPI?
• Balance: do you have a set of KPIs that measure different aspects of
the services provided?
• Tested: have national and/or international KPIs been considered?
• Safe: will an undue focus on the KPI lead to potential adverse effect on
other aspects of quality and safety?
• Timeliness: is the information available within an acceptable period of
time to inform decision makers?

• KPI Title
• 30 Days Readmission Rates
• KPI Description
• Readmissions of homecare patients are costly. This KPI will focus on the
number of homecare patients that get re-admitted to the hospital within 30
days post discharge.
• KPI Rationale
• The purpose of this KPI is decrease the number of readmission of homecare
patients within 30 days post discharge.
• KPI Target
• The target for this KPI is not to exceed 5%; that is less than 5% our homecare
patients will be readmitted to the hospital within 30 days post discharge.

• Numerator = Number of homecare patients readmitted within 30 days post
discharge
• Denominator = Total number of homecare patients cared for
• Actual Calculation: Numerator divided by the Denominator, multiplied by 100
