Diuretic Resistance <ul><li>Poor oral absorption due to gut edema </li></ul><ul><li>Decreased perfusion of kidney results in decreased delivery of loop diuretics </li></ul><ul><li>Decreased GFR results in decreased filtered load of sodium </li></ul><ul><li>Volume depletion activates plasma renin activity and stimulates SNS </li></ul><ul><li>Increased proximal tubule reabsorbtion of sodium, particularly in setting of elevated AII and elevated catecholamine levels </li></ul><ul><li>Increased distal reabsorbtion of sodium, stimulated by aldosterone </li></ul><ul><li>Associated with tubular hypertrophy: resetting basal rates of sodium reabsorbtion. </li></ul>
