This is a checklist for patients to document the staff members that fulfilled each of the mental health tasks outlined in the WPATH SOC for adults pursuing medical transition.

Wpath SOC 23 28 checklist

  1. 1. 1 WPATH SOC Pages 23-28 Checklist Pages 23-28 of the World Professional Association for Transgender Health’s Standards of Care 7th Edition describes the mental health screening, assessment, and care recommended for gender dysphoric adults. The SOC does not recommend psychotherapy as a required clinical response to gender dysphoria. (Pg.28.) It does recommend that either a mental health professional or “another health professional with appropriate training in behavioral health and with sufficient knowledge about gender nonconforming identities and expressions and about possible medical interventions for gender dysphoria” complete the following tasks. This is a checklist for you, the patient, to verify that you’re receiving the mental health screening, assessment, and treatment planning recommended by the SOC. All of these quotes are directly from the SOC, laid out in checklist format with a space for you to document what staff member completed the task and on what date. Once you sign an informed consent form to get hormones or surgery, you are relinquishing your right to sue for whatever damage accrues to you because these tasks were not completed. I’d encourage you, when choosing a clinic, to use this as a reference for discussing with the intake personnel what services you can expect them to provide. Page 23 “Mental health professionals assess clients’ gender dysphoria in the context of an evaluation of their psychosocial adjustment (Bockting et al., 2006; Lev, 2004, 2009).
