Meaningful Use in Radiology

  1. 1. Meaningful Use in Radiology What you need to know to qualify for incentives, and how Carestream can help.

What can be expected with Meaningful Use US Healthcare System – The US healthcare system is the most costly healthcare system in the world, yet according to the World Health Organization, ranks very low as far of quality of care. Legislation – To overcome its expense and short comings, the US government legislated a series of Acts. Of these Acts, the one that is of most interest is the Health Information Technology for Economic and Clinical Health Act (HITECH) HITECH – Outlines a series of programs to better healthcare through a multi-stage, multi-year plan to create a national electronic health information system. The initiative is across all of healthcare practitioners, including specialties like Radiology. Stick & Carrot – The government is taking a incentive and penalty approach. The government has put up over $27B in incentives and also billions in penalties to push for compliance.
  3. 3. What can be expected with Meaningful Use Overview – HITECH and Meaningful use is complex, but they work on a simple premise. For a healthcare IT system, you need 3 ingredients: Need to capture Information, Need to know what information to capture, Need a way to share information. Capture – The Act requires that certified software be used to capture specific healthcare data. There are 2 types of certification, an EHR or an EHR module. An EHR must manage all of the meaningful data. An EHR module manages a subset of the meaningful data. We will certify Carestream RIS as an EHR module. What Information – Having a certified EHR is not enough, you need to show that you are using the EHR in a proper way, a meaningful way. Meaningful use outlines a series data and structured data that a system has to manage. Sharing – The federal government is setting up a Health Information Exchange (HIE) for EHR’s to share information. The HIE’s are state designed and managed. The Federal Government has >$500M to assist with the infrastructure.
  4. 4. What can be expected with Meaningful Use Carrot – Healthcare professionals will receive $44K per eligible professional to follow and submit meaningful use information. So an imaging center group with 50 Radiologists would be eligible for $2.2M in incentives. Incentives – In order to receive the maximum incentive, facilities must start submitting meaningful use information for at least 90 days in 2012. Stick – Medicare and Medicaid are going to penalize all facilities that do not comply by reducing reimbursement. Private healthcare companies are expected to follow suit, and the reimbursement penalties are expected to be higher. Adoption – Initially there were questions as to how heavily meaningful use would be adopted, especially in specialties like Radiology. But States are applying to make physician board certification dependant on adoption of meaningful use. Combining board certification with private insurance company penalties, will ensure its adoption.
  5. 5. Two Possible Workflows for Compliance Responsibilities – In order for a facility to be eligible for the full incentives they must submit 90 days of meaningful data in 2012. Because of the tight timelines are dependant on the speed of the industry to implement all of these EHRs, there will be a mixture of solutions. Certified EHR – Sites that have the money and are able to implement a certified EHR in time will be safe in submitting information. But how and where does that information come from. They will want their ancillary systems to feed the meaningful data in the correct format. We need to support this.

This does not eliminate them from the meaningful use incentive. Provided they supply the meaningful data to CMS, they are eligible. We need to support this scenario.   24. 24. Three Components of Meaningful Use Data Meaningful Data – The requirements for meaningful use are broken up into three main areas; Core, Menu & CQM (Clinical Quality Measures). Some of the requirements are specific to inpatient facilities, and some to outpatient facilities. Also, some of the requirements have exemptions. Core – These represent items that facilities are required to submit, provided they do not qualify for an exemption. They make up the essential data required to create a medical record, or provide a critical benefit to patient care. Menu – A series of requirements in which a facility must present only 5 of them in stage 1 of meaningful use. There are some that are specifically for inpatient facilities and some for an ambulatory setting. CQM – Clinical Quality Measures are a series of statistical data that facilities must submit. Inpatient facility must submit all 44, while an outpatient environment must submit 9; 3 core, 3 alternate, and 3 optional.

  26. 26. How CARESTREAM RIS Can Help Certification – We will be submitting RIS for certification in the first half of 2011, and upgrading our certification in the second half of 2011. There are currently no RIS products that are certified. EHR Module – CARESTREAM RIS captures and shares meaningful use information. We are showing this at RSNA, and will be demonstrating it on the show floor. Careful Communication – HITECH and Meaningful use is very new and fairly complex. Make sure that you are clear in your communication with your customers, and seek assistance if required. Our customers will be seeking our guidance.

  29. 29. Meaningful Use Compliance With RIS (some examples) Referring – Track which Meaningful Use data has been captured by the referring physician. The facility can then record any missing information in the RIS. Vital Signs – Manage the patients vital signs including automatic calculation of the Body Mass Index (BMI) and trending the vital signs over time.
