What is CDSMP<br />Evidence based, peer-led programming designed to help people gain self-confidence in their ability to control their symptoms and health conditions and how their health problems affect their lives.<br />Small group workshops include six 2½-hour sessions<br />4 program offerings<br />Chronic disease SM(English & Spanish)<br />Diabetes SM (English & Spanish)<br />

Statewide Goals for CDSMP <br />Increase availability of CDSMP to older adults in Colorado<br />Establish a statewide CDSMP distribution and delivery system<br />Maintain quality through fidelity to the evidence based program<br />Develop partnerships to build CDSMP implementation and sustainability<br />
  4. 4. Timeline and Targets<br />Grant period March 30, 2010 – March 30, 2012<br />Target population = people with chronic diseases > 60 years old<br />Target = 566 participant completers <br /> (4 of 6 workshop sessions)<br />

University-School of Public Health Role<br />Hold Stanford license<br />Maintain 2-3 certified Master Trainers <br />Coordinate statewide master and lay leader trainings<br />Ensure trainers have opportunities to conduct workshops<br />Report leader licensing to Stanford<br />Perform fidelity checks <br />Report to CDPHE<br />
  10. 10. Local Lead Agency Role<br />Assess local population needs<br />Identify and recruit host sites<br />Determine which programming best serves local populations<br />Market programs<br />Identify potential lay leaders<br />Ensure program fidelity, sustainability<br />Report to CDPHE<br />
