This does not meanthat the senior will not need home care for a lengthy period but there isa window after discharge where the risk of poor outcomes, including ofreadmission is highest. The goal of a transitional care program is toprovide both care and education during this window to help ensure thatthe patient is safe and continues on the road to recovery.

As a matter of fact,we can reduce required travel by doing shopping and picking upprescriptions. We can also provide transportation to Doctor’s and otherappointments, including hair and nail appointments. This can greatlyreduce the stress on the senior living at home.
