SAC/BPR Implementation: Health Care and Business Plans <ul><li>Effective with SAC and BPR submissions for FY 2009, BPHC will redesign the Health Care Plan (HCP) and the Business Plan (BP) </li></ul><ul><ul><li>HCP will include six Clinical Measures as well as one Behavioral Health and one Oral Health </li></ul></ul><ul><ul><li>May include other life-cycle based measures </li></ul></ul><ul><ul><li>BP must include two UDS and three audit financial measures </li></ul></ul><ul><ul><li>May include other business goals and objectives including governance </li></ul></ul>

SAC/BPR Implementation - 2 <ul><li>Plans will be built around a QI model </li></ul><ul><ul><li>SAC plans will state current baseline value for the measure (if available) and establish a quantified goal for the project period </li></ul></ul><ul><li>Measurement of baselines and/or progress will be automatically entered into future grant submissions once the UDS is in the EHB </li></ul>

UDS Implementation Reporting Requirements <ul><li>The 2008 UDS will have two new tables to report on the clinical measures: </li></ul><ul><ul><li>Table 6B: Quality of Care Indicators </li></ul></ul><ul><ul><ul><li>Report out on prenatal care, immunizations, and pap test </li></ul></ul></ul><ul><ul><li>Table 7: Health Outcomes and Disparities </li></ul></ul><ul><ul><ul><li>Report out on hypertension, diabetes, and infant birth weight </li></ul></ul></ul><ul><li>Grantees will use random sampling methodology for these reports </li></ul><ul><ul><li>70 charts per measure </li></ul></ul>

Consultative Role for the Primary Care Association <ul><li>PCAs should be prepared to answer questions about the new HCP and BP process </li></ul><ul><li>PCAs should be able to clarify </li></ul><ul><ul><li>What the new HCP and BP measures are </li></ul></ul><ul><ul><li>How are baseline data to be obtained and/or what to do if applicants cannot obtain baseline data </li></ul></ul><ul><ul><li>How will the UDS data be integrated into the HCP / BP process. </li></ul></ul>
