  • 1. Outcomes Monitoring Framework – FASS Link Worker Service, Overall Aims To offer a confidential Kinship Care service to adults affected by a family members’ alcohol / drug addiction Activity 1: To sign-post Kinship Carers to identified services based on need Specific Aims Outcomes Outcome Indicators Data Collection Methods 1. To inform of & increase referrals to FASS & the Kinship Care service 6a) Organisations & workers have; Extent to which organisations & workers report they have; 6.1 increased knowledge of the services & supports that FASS & the Link Worker in particular provide 6.2 an increased knowledge & understanding of the Link Worker’s partnership role within FASS & Geeza Break & the 2nd Generation Care service criteria 6.3 know the referral process for the Link Worker support service 6.4 increased confidence that they can refer or sign-post Kinship Carers to the Link Worker service
