20. RMN VS TOMOGRAFIA
• RMN es superior en detectar cambios
asociados a TBC SNC.

quirurgica)
• Signos mas frecuentes.
Hidrocefalia, agrandamiento basilar.
• Edema, Hipodensidades(infarto), masas.

tratamiento.(historia clinica, signos y sintomas
sugestivos, imágenes y LCR)
• Duracion 12 m
• Esquema empirico: INH. RMP.PZA.ETM
• Asociado a Corticoide.

32. CORTICOIDES
• Estudios en modelos animales demuestran que el
M.tbc daña mas la microglia(macrofagos) MAYOR
RESPUESTA INFLAMATORIA.
• Cuando se incuba celulas de microglia infectadas con
TBC asociadas a DEXAMETASONA se ha observado
una INHIBICION SIGNIFICATIVA de los mediadores de
la inflamacion.

expression by human microglia and astrocytes: effects of dexamethasone,” Journal of Infectious
Diseases, vol. 192, no. 12, pp. 2054–2058, 2005
33. CORTICOIDES
¿Afectan penetrancia ATB?
• Pequeños estudios han demostrado que no
afectan la penetrancia de las drogas de
primera linea.

steroids on cerebrospinal fluid penetration of antituberculous drugs in tuberculous
meningitis,” Clinical Pharmacology and Therapeutics, vol. 49, no. 1, pp.
6–12, 1991.
34. CORTICOIDES
• ¿ Reducen la mortalidad asociada a MEC TBC?
• Meta análisis de 1140 que incluyeron Niños y
Adultos con MEC TBC sin HIV reporta un RR de
0.69(IC 95 : 0.52-0.92).
• NO afecta de manera significativa el grado de
discapacidad o secuela neurológica.

37. ¿TERAPIA BIOLOGICA?
• Anti TNF- alfa.
• En modelos animales la asociacion de
Thalidomida(POTENTE inhibidor TNF) a los
anti TBC redujo en un 50% la mortalidad
asociada a MEC TBC.

38. RESPUESTA PARADOJAL
• 10% de casos, 30% en HIV.
• Sind de reconstitucion inmune que se traduce
en una HIPERESPUESTA INFLAMATORIA a
patógenos oportunistas.
