  2. 2. Click on Add an activity…Select Feedback
  3. 3. Fill in the requiredinformation (Name andDescription), and set the time the activity will open and close (if desired).
  4. 4. Set the Feedback options. Anonymous responses can only be used if the activity appears on the front page of the course for guests that are not logged-in. Decide if administrators will receive a notification of responses via email, if multiple submissions are allowed (must be set to yes for anonymous surveys), and if you want to automatically number each question.
  5. 5. Add a message for participants once they have completed the activity. The participant willautomatically be directed back to the home page of thecourse. If you would like to re-direct users, add the URL here.
  6. 6. Set the commonmodules settings and clickSave and display
  7. 7. Click the Edit questions tabto choose a questions type
  8. 8. Choose a question type, thenclick Add question to activity.
  9. 9. Or select Templates
  10. 10. You can use previously used questions as a template, or you can save the currentquestions as a templateand use them for future surveys.
  11. 11. When you are ready The Show responses and non-for the results, click respondents tabs, lets you seeon the Analysis tab. the individual responses or a list The results will be of course participants that havedisplayed. They can not responded.also be exported to Excel.
  12. 12. What will the students see? When participants click on the activity, they will be directed to an introductory page. To proceed to the survey, they need to click on Answer the questions…
  13. 13. Answer the questions andclick Submit your answers
  14. 14. Click Continueand you will be returned to the class
