Definition of Risk
 the potential harm that may arise from some
present process or from some future event
Types of Risk
 Clinical Risk is any issue that may have an impact
on the achievement of high quality, safe and effective
care for patients;
 Non Clinical Risk is any issue that may have an
impact on the achievement of the statutory and legal
duties of the Trust;
 Financial Risk is any issue that may have an impact
on financial objectives;
 Organizational Risk is any issue that may have an
impact on organizational objectives.
 Significant Risk is any systematic failure resulting in
harm to patients, staff or the public, r failure to
achieve performance targets after initial corrective
action, or any incident which threatens the Trust's
ability to undertake routine business.
What is Risk Management?
 Risk Management is the process of reducing the
incidence of preventable injuries in order to
minimize the financial loss to the institution
should an injury occurs.
 Risk Management is the process of measuring, or
assessing risks and then developing strategies to
manage these risks.
Goal of Risk Management
 To protect the organization from the financial
losses which may arise as a result of the risks to
which it is exposed.
Functions of Risk Management
Decrease severity and number of patient and visitor
injuries by:
 receiving and reviewing incident and occurrence
reports, as well as patient/visitor complaints
 working closely with quality improvement
committees
 periodically reviewing credentialing procedures
being involved in the education of medical staff
and employees via grand rounds, in-services and
other venues
 Assure that documentation of care is adequate by
working closely with medical record committee
educating medical staff and employees
 Limit financial loss related to clinical care and
provide a mechanism to deal fairly with issues
related to claims from adverse outcomes in
clinical care
 Investigates professional liability claims (i.e.,
malpractice) and negotiates fair resolution
manages certain insurance policies which have
been secured by the organization and its
employees

  • 3. Quality Defined  doing the right things right at the first time and every time.  Quality is also… Incremental and Continuous Improvement
  • 4. Errors can only be reduced by focusing on the design of the work itself rather than the workers 4 (Hume 1999)
  • 5. The Challenges of not having quality processes in place: 1. Infection rates 2. Medication errors 3. Readmissions 4. Complications 5. Delays 6. Unnecessary complexity 7. Variation in practice

  • 9. Definition of Risk  the potential harm that may arise from some present process or from some future event
  • 10. Types of Risk  Clinical Risk is any issue that may have an impact on the achievement of high quality, safe and effective care for patients;  Non Clinical Risk is any issue that may have an impact on the achievement of the statutory and legal duties of the Trust;  Financial Risk is any issue that may have an impact on financial objectives;  Organizational Risk is any issue that may have an impact on organizational objectives.  Significant Risk is any systematic failure resulting in harm to patients, staff or the public, r failure to achieve performance targets after initial corrective action, or any incident which threatens the Trust's ability to undertake routine business.

What is Risk Management?  Risk Management is the process of reducing the incidence of preventable injuries in order to minimize the financial loss to the institution should an injury occurs.  Risk Management is the process of measuring, or assessing risks and then developing strategies to manage these risks.
  • 12. Goal of Risk Management  To protect the organization from the financial losses which may arise as a result of the risks to which it is exposed.
  • 13. Functions of Risk Management Decrease severity and number of patient and visitor injuries by:  receiving and reviewing incident and occurrence reports, as well as patient/visitor complaints  working closely with quality improvement committees  periodically reviewing credentialing procedures being involved in the education of medical staff and employees via grand rounds, in-services and other venues
  • 14.  Assure that documentation of care is adequate by working closely with medical record committee educating medical staff and employees  Limit financial loss related to clinical care and provide a mechanism to deal fairly with issues related to claims from adverse outcomes in clinical care  Investigates professional liability claims (i.e., malpractice) and negotiates fair resolution manages certain insurance policies which have been secured by the organization and its employees
  • 15. Importance of Risk Management  Minimize the adverse effects of losses as much more than just a financial way.  Consider regulatory compliance  Safety management  Credentialing  Client-provider relations  Publicity and media coverage  Client care
  • 16. Steps of Risk Management Three R’s and five I’s:  Risk Identification  Risk Analysis  Risk Control/Treatment  Investigate  Inform  Influence  Interpret  Integrate
  • 17. Risk Assessment  Risk assessment is the process of:  – Identifying Hazards  – Analyzing or evaluate the risk associated  with that hazard  – Determining appropriate ways to eliminate,  reduce or control the consequences of the  risks
