  • 3. Personal Health Record Overview Provides a web-based portal for patients to review and contribute to their medical information. Portable: Medical information can be accessed at any location and shared with providers, specialists, and more. Interactive: Patients may contribute information which may benefit their care, such as blood glucose or pressure, and share information with providers. Efficient: Better information quality and exchange can result in improved and proactive care, as well as cost savings.
