  • We “WakeMeditized” the Institute’s pinwheel logo as well as the core concepts of Patient & Family Centered Care. In order to effectively incorporate our care delivery system at WakeMed, we need to have all the PFCC core concepts working together – just like a pinwheel needs all of its parts to work.
  • Notice this refers to All members of the WakeMed team. We are beginning the rollout of PFCC with Nursing; it will then be rolled out to the entire health system. Nurses will serve as role models for PFCC, helping other employees put themselves in our patients’ and families’ shoes.

    6. 6. So, we’re here to help! • Patient & Family Centered Care (PFCC)! • PFCC is our Nursing Care Delivery System. It’s based on the fact that patients and their families have the right to know everything you know about the patient’s care and condition. • It’s a partnership to ensure we are meeting the specific and individualized needs of each patient and their families. 6
