prolonged sore spot(ulcer) on the tongue

Most tongue cancerpatientsareadmitted whenthey’reatthefinal stagesof cancer. Everysooften,
theywould mistaketonguecancerforlessserious medical conditionslikecommonmouthsoresor
glossitis.
Mr. B.V.T(aged 55, Tay Ninh) self-medicated a sore spot onhis tongue in 4months buthis
conditionremained the unchanged.Afterbeing examined by the doctor,he was thoughttohave
tonguecancer,thuswas referred to HCMCOncologyHospital fora more accuratediagnosis. After
getting a biopsy, he wasdiagnosed with tonguecancerand wasrushed to the operation roomfor a
glossectomy to preventmetastasis (thespread ofcancerfrom one organor part to another)
Mr. H.T.V(aged 47,LongAn) initially experienced persistent sores on the mouthfollowed by tongue
ulcers.He had been receiving medical treatment for3 monthsbut nocancerousconditionwas
detected. Suspectingthat Mr. T might be having oral cancer,his family broughthim to HCMC
OncologyHospital to be thoroughlyexamined. Aftergetting a biopsy, he wasdiagnosed with
tonguecancer.
Men face a higher risk ofdeveloping tongue cancer than women
AccordingtoDr.Vo DangHung,Ph.D.,Directorof TMMCHealthcare’s Oncologycenter,tongue
canceris the most commontype oforal cancer.It stems from the malignant transformation ofthe
tonguetissues. It is most likely amongolder adults (fromthe age of50 to 60). Menface a higher risk
ofdeveloping tonguecancerthan women.
To better explain whymen are exposedto a higher risk, Dr. Hungstated: “The risk factorsof tongue
cancerinclude unhealthy habits like smoking, excessive intake ofalcohol and frequentbetel
consumption.However,the numberof men whosmoke or taking alcohol is muchgreater that that
ofwomen whoconsumebetel, thus,men are at an increased risk fordeveloping tonguecancer,”
AccordingtoDr.Hung,alcohol, tobaccoandbetel leaves inducetoxic chemicals that directly
damage out tongues.These damages canlead to cancerif left untreated. Besides, other tongue
injuries (dueto the teeth or some external factors),throatinfectionor other bacterial oral infections
canalso lead to tonguecancerif not treated promptly.Without early detection, the cancerwill
spread to other organsor parts like bones,lung,liver etc. and may eventually lead to death.
“Inmost cases, the patient visits the hospital during the final stages of cancer(Stage3-4) and isn’t
aware that he/she has cancerbut thinks that it’s justanother case oforal infection.Inthese cases,
we have to remove a part ofthe patient’s tongueor worse,all of his/her tongue. This will
permanently changethe patient’s ability to speak and swallow.Ifthe conditionis detected and
treated sooner,the outcomewill be muchbetter!” – said Dr.Hung.

Pay attention to oral hygiene to prevent tongue cancer
Ifyou experience any ofthese symptoms: swelling or lumps/cystson the neck, a sore spot(ulcer) on
the tonguethat doesnot go away,a red or white patchon the tongue,unexplained bleeding, rash
or pain onthe tongue, difficultychewing,swallowingor speaking etc., youshouldvisit the doctoras
soonas possible. These are the common symptomsof tonguecancer.
“Providingthat tonguecanceris early detected, physicians will be able to remove the tumor
withoutremoving half or all ofthe patient’s tongue.Inthese cases,the patient canget a
reconstructivesurgery,otherwise, it is impossible for the tongueto be reconstructedsince
Vietnamese hospitals are incapable of performingsuchsurgery.There are a few successfulcasesin
the world butthe chancesare still slim. This is oneof the challenges in tongue cancertreatment.”
Said Dr. Hung.
AccordingtoDr.Hung,the best ways toprevent tongue cancerincludegiving upsmoking & betel
consumptionas well as refraining from drinkingalcohol. Ifyou experience tongueinfection and
have been treated in 10-15daysbut the conditionremains unchanged,consultaspecialist. Also,pay
attention to oral hygiene to prevent yourselffrom developing tonguecancer.
According to a research conducted by Dr. Nguyen Huu Phuc, Nguyen Chan Hung
and Tran Van Thiep on 310 tongue cancer patients treated at HCMC Oncology
Hospital from 1999 to 2011, the average age for developing tongue cancer is 57,9
and the ratio of males to females is 1.7:1. The percentage of patients admitted at
the final stages (3, 4) is 61.6%. The 3-year survival rate after treatment is 47%
(70.9% for stage 1, 2; 28.8% for stage 3, 4)

Đa số bệnhnhânung thưlưỡi đềunhập viện ởgiai đoạncuối và nhầmtưởngmìnhchỉbị nhiệt,viêm
hoặcloét vùng miệng.
ÔngBVT (55tuổi, ở Tây Ninh) tự điều trị một vùngloét nhỏ bênbờ phải củalưỡi bốntháng không
khỏi nên đến bác sĩ khám. Sau khi xem quavùngloét, bác sĩ nghi ngờôngbị ungthư lưỡi nêngiới
thiệu ôngđến BV UngbướuTP.HCMđể khám. Bệnh viện tiến hành lấy sinh thiết và kết luận ôngbị
ungthư lưỡi, phải cắt bỏ vùngbị ungthư,nếu khôngsẽ di cănsangcác vùngkhác.
ÔngHVT (47tuổi, ởLongAn) ban đầubị nhiệt miệng, sauđó lưỡi bị loét. Ôngđã điều trị nội khoa
ba tháng ởmột bệnh viện nhưngcác bác sĩ khôngphát hiện bệnh ungthư.Nghi ngờ bị ungthư lưỡi
nên ngườinhà đã đưa ôngđến BV Ungbướukhám. Sau khi làm sinh thiết, các bác sĩ khẳngđịnh
ôngT. đã bị ungthư lưỡi.
Nam giới dễ bị hơn phụ nữ
Theo bác sĩ VõĐăng Hùng,Trưởngkhoa Ngoại V, BV UngbướuTP.HCM,ungthưlưỡi là bệnh
thườnggặp nhiều nhấttrong các ungthưvùngmiệng. Nóphát sinh từ sự biến đổi ác tính biểu mô
phủ lưỡi hoặc các mô liên kết cấutrúc lưỡi. Đây là bệnh thườnghay gặpở người già (độtuổi từ 50
đến 60).Nam giới dễ bị ungthư lưỡi hơnphụnữ.
Lý giải nguyênnhân nam giới dễ bị hơn,bác sĩ Hùngchobiết: “Các thói quen cónguycơgây ung
thư lưỡi cao là uốngrượu,hútthuốc lá, nhai trầuvà xỉa thuốc.Tuy nhiên, sốnam giới hút thuốc lá,
uốngrượunhiều hơnso vớisố phụ nữ ăn trầu,xỉa thuốc nêntỉ lệ nam giới bị bệnh sẽ cao hơn”.
Theo bác sĩ Hùng,rượu,thuốc lá, trầusẽ tạo ra những chấtkích thíchvà hóa chấtđộc hại tác dụng
trực tiếp lên niêm mạc vàlàm tổn thươnglưỡi. Những tổnthươngnày nếu kéo dài cóthể gây ung
thư. Ngoài ra, nhữngchấnthươngcủalưỡi (dorănghoặc do tác độngtừ bên ngoài),hoặc bị viêm
vùngmiệng, họng…lâu ngày nhưngkhôngđiều trị cũngchuyểnsangungthư lưỡi. Nếu không phát
hiện sớm cóthể di căn sangcác bộ phậnkhác nhưxương,phổi, gan…và cóthể dẫn đến tử vong.
“Đa số bệnhnhân thườngnhậpviện ở giai đoạnmuộn(giai đoạnIII,IV) và khôngbiết mình bị ung
thư lưỡi vì cứ tưởngbị nhiệt miệng hoặc viêm loét vùnglưỡi. Khi đó, chúngtôibuộc phải cắt bỏnửa
lưỡi hoặc nặnghơnthì phải cắt bỏ toàn bộlưỡi củabệnhnhân. Điều này sẽ ảnh hưởngđếnchức
năngnói và nuốtthức ăn củangườibệnh. Nếu nhưbệnh nhânphát hiện bệnh sớm hơnvà đến các
cơsở y tế để khám thì sẽ khôngdẫn đếnhậu quảđáng tiếc như vậy!”- bác sĩ Hùngbày tỏ.
Vệ sinh răng miệng để phòng ngừa
Khi bệnh nhângặp một trongnhữngbiểu hiện như: xuấthiện hạch ởcổ,vết loét lâu ngày, cóvệt
màu trắng hoặc đỏở hai bên lưỡi, chảy máu lưỡi, ngứa hoặc đaurát lưỡi, cókhối u ởvùnglưỡi hoặc
khó khăn khi nói, nhai…thì nêngặp bác sĩ. Khôngloại trừ một trong nhữngbiểu hiện trên là triệu
chứngcủaungthư lưỡi.
“Nếu phát hiện ungthư lưỡi sớm, các bác sĩ sẽ cắtbỏ phầnu ungthư đúngmức,sau đótạo hình lại
cholưỡi. Riêng đối với nhữngbệnh nhânbị cắt nửa lưỡi hoặc toàn bộ phầnlưỡi thì hiện chưacó

bệnh viện nàoở Việt Nam cóthể tạo hìnhđược.Trên thế giới đã có một sốnước thực hiện việc này
thành côngnhưng phươngphápthì chưađược phổbiến rộngrãi. Đây là khó khănhiện nay trong
côngtác điều trị” -bác sĩ Hùngchobiết.
Theo bác sĩ Hùng,biện phápphòngngừa ungthư lưỡi tốt nhất là mọi ngườinên bỏthói quenhút
thuốc lá, ăn trầu, xỉa thuốc vàhạn chế uốngrượu.Nếulưỡi bị viêm, loét và được điều trị đúngcách
trongvòng10-15ngày nhưngchưakhỏi thì bệnh nhânnên đến gặp các bác sĩ chuyênkhoa để khám
bệnh và hướngdẫnđiều trị. Việc vệ sinhrăng miệng sạch sẽ cũnggiúpphòngngừacác bệnh về răng
miệng và giảm thiểu nguycơungthư lưỡi.
Một nghiên cứu củanhóm bácsĩ Nguyễn Hữu Phúc, Nguyễn Chấn Hùng và Trần
Văn Thiệp với 310 bệnh nhân bị ung thư lưỡi tại BV Ung bướuTP.HCM từ năm 1999
đến 2001 cho thấy: Độtuổi trung bình của bệnh nhân bị ung thư lưỡi là 57,9; tỉ lệ
nam/nữ là 1,7/1. Bệnh nhân nhập viện muộn (giai đoạn III, IV) là 61,6%. Tỉ lệ người
bệnh còn sống sau ba năm điều trị là 47% (điều trị ở giai đoạn I, II là 70,9%; giai
đoạn III, IV là 28,8%).
