    • LP Claim Form Homes Built Before Jan 1996
    • Previous Claims
      Check Yes if you have filed a previous claim
      If you have not move to section A.
      Fill in the year that you filed the claim along with about how much you think you have received.
      Check original settlement fund.
    • Section A
      - Fill in your street address along with the city, state, and zip code.
      - The next section fill out the information for all the homeowners on the title of the home.
    • Section B
      Check if your home was new or previously owned. If it was previously owned put the year that you bought your home.
      The next thing you need to write down is the number one where is asks number of structures.
    • Section B Continued
      For type of structure check single family dwelling.
      For type of LP check siding and trim
    • Sections C, D, and E
      C: You need to provide a warranty deed, paid tax receipt, and a current utility bill. If you need help on how to find your warranty deed refer to slide #11
      D: You need to prove at least two pictures of the LP knot. If you need help you can set up an appointment and we can have a rep come out and help you.
      E: You need to put down the year your home was built and the document you need to provide is a certificate of occupancy. Refer to slide #11 for help on how to get a certificate of occupancy.
    • Section F and G
      F: You need to put down that you have painted at least 5-6 years ago or they will hit you with paint deductions and take money from your claim.
      G: Skip
    • Section H, I, and J
      H: For section H provide directions from the nearest highway to your home
      Check yes that you would like to be present for the inspection when the state inspector comes out to inspect your siding after your claim form has been sent in.
      I: Skip
      Next all homeowners need to sign, print, and date at the bottom of the page.
    • Questionnaire
      Fill out all your information is section A
    • Tax information
      Answer No to all 3 tax questions and since all 3 are no you do not have to provide your social security number.
      The last thing you need to do is have all homeowners sign, print, and date the claim form.

      • Click “Official Public Records”
      • Search under Grantee (your last name first), select Warranty Deed under document type, and type your city in Subdivisions and then click search
      Johnson County
      • To receive a warranty deed you must either go to the Records office or mail in a request with Page numbers and book number of deed (they can call in and ask them to look up the page numbers- $1 per page) then you must mail it into:

      • On left hand side click Online Access and then click it again in the drop down
      • Click Online Access Systems on top
      • Login as GUEST
      • Search under first and last name
    • Paid Tax Receipt
      Jackson County
      Go to www.jacksgov.org
      Scroll to the middle of the paid and on the left hand side you will see a link that says Taxes, under that link you will see another one that says Print Tax Receipt. Click on it.
      You will use the second search box that says “Owner Name Search”. Be sure to put your Last name first followed by your first name with an asterisk next to it.
      Your parcel number will be the longest number with all the dashes in between. (xx-xxx-xx-xx-xx-x-xx-xxx)
      Once you click on your parcel number it will bring up your paid tax receipt; you can print this off and use it as #6 (page 1) for option A ; copy of property tax bill

      • Click on the top menu bar that says “Real and Personal Property Taxes”
      • On the left hand side click “View and Pay Tax Bill”
      • A disclaimer page will pop up and you need to click yes.
      • Search under Real Estate on the Left hand side
