Requirements for Demonstration of Meaningful Use <ul><li>Must achieve compliance with 5 out of 10 Menu Set measures* </li></ul><ul><ul><li>One of the five Menu Set measures must be related to Public Health (either Immunization Registry or Electronic Syndromic Surveillance) </li></ul></ul><ul><li>* Some exclusions are allowed for specific measures, reducing the number of required measures accordingly </li></ul>

Drug Formulary Checks: Menu Set Measure #1 <ul><li>The EP has this functionality and has access to at least one internal or external formulary for the entire EHR Reporting Period. </li></ul><ul><li>Stage One Threshold: Attestation </li></ul><ul><li>Exclusion: Any EP who writes fewer than 100 prescriptions during the EHR reporting period </li></ul>

Lab Results: Menu Set Measure #2 <ul><li>Incorporate clinical lab test results into EHR as structured data. </li></ul><ul><li>Stage One Threshold: > 40% of all clinical lab test results ordered by the EP during the reporting period whose results are either in a +/- or numerical format are recorded as structured data </li></ul><ul><li>Exclusion: An EP who orders no lab tests whose results are either in a +/- or numeric format during the EHR reporting period </li></ul>

  10. 10. Patient Lists: Menu Set Measure #4 <ul><li>Generate at least one report listing patients of the EP with a specific condition to use for QI, reduction of disparities, research or outreach </li></ul><ul><li>Stage One Threshold: attestation </li></ul><ul><li>No exclusion. </li></ul>

Patient Reminders: Menu Set Measure #4 <ul><li>Send reminders to patients per patient preference for preventive/follow-up care. </li></ul><ul><li>Stage One Threshold: >20% of all patients 65 years or older or 5 years or younger were sent an appropriate reminder during the EHR reporting period. </li></ul><ul><li>Exclusion: EP who has no patients >= 65 or <= 5 years of age with records maintained in the certified EHR technology. </li></ul>

Timely Access: Menu Set Measure #5 <ul><li>Provide patients with timely electronic access to their health information (lab results, problems, Rx, allergies) within 4 business days of the information being available to the EP. </li></ul><ul><li>Stage One Threshold: > 10% of all unique patients seen are provided timely electronic access </li></ul><ul><li>Exclusion: EP who have no such patient data to provide during the reporting period </li></ul>

Patient Education: Menu Set Measure #6 <ul><li>Use certified EHR technology to identify patient-specific education resources and provide those resource to the patient if appropriate </li></ul><ul><li>Stage One Threshold: > 10% of unique patients seen during the reporting period are provided Patient Education resources </li></ul><ul><li>No Exclusion </li></ul>

Transition of Care Summary: Menu Set Measure #8 <ul><li>EP who transitions or refers their patient to another setting of care or provider of care provides a summary of care record </li></ul><ul><li>Stage One Threshold: > 50% of transitions of care/referrals </li></ul><ul><li>Exclusion: EP who neither transfers a patient to another setting nor refers a patient to another provider during the EHR period. </li></ul>

Immunization Registries Data Submission: Menu Set Measure #9 <ul><li>Capacity to submit electronic data to immunization registries and actual submission according to applicable law and practice </li></ul><ul><li>Stage One Threshold: Perform at least one test of certified EHR’s capacity to submit (submission if registry has capacity to receive) </li></ul><ul><li>Exclusion: EP who administers no immunizations during the EHR reporting period or where no registry has capacity to receive electronically </li></ul>

Electronic Syndromic Surveillance: Menu Set Measure #10 <ul><li>Capacity to submit ESS data to public health agencies according to applicable law and practice </li></ul><ul><li>Stage One Threshold: Perform at least on test of certified EHR’s capacity to submit ESS (unless no Public Health Agencies have capacity to receive info electronically </li></ul><ul><li>Exclusion: EP who does not collect/submit any reportable syndromic information on their patients </li></ul>
