Comparison of the cutting balloon with high pressure dilation of segmental pulmonary artery stentosis. (contingent CHR approval pending final modifications). This is a multi-center FDA approved randomized trial evaluating a new balloon dilation catheter with small razor blades which score the vessel intima and improve the ability to dilate stenotic blood vessels. Segmental pulmonary artery stenosis seen in congenital heart disease are particularly resistant to balloon dilation repair with limited other therapeutic options. Patient enrollment should begin 9/04. 2. “Evaluation of the safety and efficacy of the septal repair device in patients with secundum atrial septal defect (ASD)”(H7276-13240-06A). This is an FDA 22
  • 23. approved multicenter prospective clinical trial evaluating catheter device closure of atrial septal defects in children and adults. New patient enrollment is complete with final follow up data just finishing. 3. “Evaluation of the safety and efficacy of septal repair device closure of intracardiac defects in high risk patients”( H7276-14063-05A). This is an FDA approved multicenter prospective clinical trial evaluating catheter device closure of intracardiac defects, such as fenestrated Fontan, muscular VSDs, or large PDAs, in patients who are high risk for surgical repair. New patient enrollment is complete as is follow up data. Studies 1 and 2 have been instrumental in FDA approval of ASD and PFO device closure that is rapidly becoming the therapeutic procedure of choice for treatment replacing more traditional and costly surgical therapies. 4. “MRI evaluation of coarctation before and immediately after stent repair : Effect on collateral flow”. This is a prospective clinical study being done in conjunction with the department of Radiology assessing the immediate effects of stent repair of coarctation on blood flow through collateral vessels. Enrollment is complete. Patients will now be reevaluated at 1 and 2 year follow up for vascular changes related to stent implantation. B. Active Animal / Physiology Research: 1. "Effects of pulmonary insufficiency and stenosis on bi-ventricular function” (A7276-16580-01-03). We are currently in stage three of this protocol having completed evaluation of isolated insufficiency and combined insufficiency and stenosis effects on biventricular function in growing swine. We will now evaluate the timing of ventricular injury and its reversibility after the development of insufficiency using a transvenously implantable valved stent. Animals experiments for the third stage will begin this winter. 2. “Stent placement in the ductus arteriosus to provide controlled pulmonary blood flow in an animal model” (A7276-20213-01). This study evaluates placement of a newly developed covered stent in the ductus arteriosus of newborn piglets in an attempt to maintain ductal patentcy and limit pulmonary blood flow to safe levels for up to three months. This has potential application to infants with complex heart diseas e to reduce their need for surgery. This protocol includes in-vitro modeling as well as in-vivo stent implantation. Preliminary work has been done in vitro and in-vivo. This study is in progress. 3. “Magnetic Resonance Evaluation of Coarctation of the Aorta after Percutaneous Stent Repair in a Swine Model (A7276-24340-01). This study evaluates ventricular – arterial coupling and the effect of acute relief of aortic obtruction with stent repair. It also evaluates the ability to perform stent repair solely under MRI guidance. This study is in progress.
